摘要
目的检测子宫内膜异位症(EMS)患者术前术后血清中甲壳质酶蛋白40(YKL-40)、C-反应蛋白(CRP)的浓度,探讨其在子宫内膜异位症诊疗中的临床价值。方法选取2013年11月-2015年11月在本院经临床和病理证实为EMS的患者100例为EMS组,选取同期住院的80例卵巢单纯囊肿的患者作为对照组。采用酶联免疫吸附试验(ELISA)检测EMS组术前术后及对照组的血清中YKL-40、CRP的浓度,并检测EMS不同分期血清YKL-40和CRP的浓度。结果 EMS组血清YKL-40和CRP的浓度均明显高于对照组,差异均有统计学意义(t值分别为8.30、9.74,P<0.01);EMS组中3/4期患者血清YKL-40的浓度明显高于1/2期,差异有统计学意义(P<0.05);EMS组中1/2期和3/4期患者的血清YKL-40和CRP的浓度均明显高于对照组,差异有统计学意义(P<0.05);EMS组手术前血清中血清YKL-40、CRP的浓度明显高于手术后,差异有统计学意义(P<0.05)。结论血清YKL-40和CRP有望成为EMS诊断和临床治疗疗效判断的参考指标,且对EMS患者的血清YKL-40浓度的测定能够进一步提示临床分期。
Objective To test the serum concentrations of YKL-40 and CRP in patients with endometriosis before and after surgery,so as to explore the clinical value of YKL-40 and CRP in the diagnosis of endometriosis. Methods From November2013 to November 2014,a total of 100 cases with EMS verified in clinical and pathology were included into this study as the EMS group. At the same time,another 80 cases with simple ovarian cyst were enrolled as the control group. Enzyme-linked immunosorbent assay( ELISA) technique was applied to detect serum concentrations of YKL-40 and CRP in EMS group and that in different stages. Results The serum concentrations of YKL-40 and CRP in endometriosis group were both significantly higher than those in the control group,with the differences statistically significant( t = 8. 30,9. 74,P〈0. 01). The serum YKL-40 levels in stage 3 /4 of EMS group were significantly higher than those of stage 1 /2,with the difference statistically significant( P〈0. 05). The serum levels of YKL-40 and CRP in stage 1 /2 and stage 3 /4 were significantly higher than those of the control group,with the difference statistically significant( P〈0. 05). The serum levels of YKL-40,CRP of EMS group in presurgery were higher than that in postsurgery,and the differences had statistical significance( P〈0. 05). Conclusion Serum YKL-40 and CRP are expected to be a reference index for the diagnosis and clinical treatment of EMS,and the determination of serum YKL-40 concentration in EMS patients can further preview the clinical stages.
