摘要
目的分析老年慢性阻塞性肺疾病（COPD）患者缺氧程度与血清肾损伤指标水平的内在关系,从而为老年COPD患者肾损伤建立检测指标。方法随机抽取2013年5月~2015年12月在本院就诊的160例老年COPD患者,按照是否缺氧程度分为A、B两组,再按照缺氧程度进一步将B组分为B_1,B2和B3三个亚组,另选取40例年龄相仿的健康老年人作为对照组（C组）,采血并检验血清微球蛋白（α_1-MG和β_2-MG）、血清胱抑素C（Cys-C）及肌酐（Scr）水平,观察各指标差异及相关性。结果 COPD老年患者血清α_1-MG,β_2-MG,Cys-C水平明显高于健康老年人,差异显著（P〈0.05）;B_1组Cys-C水平显著高于A组,但两者其他指标无显著性差异（P〉0.05）;B2组α_1-MG,β_2-MG,Cys-C均高于B_1和A组（P〈0.05）;B3组α_1-MG,β_2-MG,Cys-C均显著高于A组、B_1组、B2组（P〈0.05）;各组间Scr水平无显著性差异（P〉0.05）。结论老年COPD患者肾损伤与缺氧程度密切相关,可以通过检验α_1-MG,β_2-MG,Cys-C对肾损伤程度进行监测。
Objectives To analyze the relationship between the level of hypoxia and the level of renal injury in elderly patients with chronic obstructive pulmonary disease（ COPD）,and to establish the detection index of renal damage. Methods Chose 124 cases of elderly patients with COPD from 2013 may to 2015 December in our hospital,according to the degree of hypoxia,they were divided into A,B,C three groups. Select another 40 cases of age-matched healthy elderly people as control group（ Group D）. Inspect the level of microspheres proteins（ alpha 1-mg and 2-mg）,serum cystatin C（ Cys） and creatinine（ SCR）,Observed the differences and correlations among them. Results COPD in elderly patients with alpha 1-mg and beta 2-mg,Cys-C levels were significantly higher than that of healthy elderly（ P〈 0. 05）; The level of Cys-C in B1 group was significantly higher than that of A group（ P〈 0. 05）,but there was no significant difference between other indicators（ P〉 0. 05）; Cys-C,alpha 1-MG,beta 2-MG of group B2 were higher than B1 and A group（ P〈 0. 05）;,alpha1-MG,Cys-C,beta 2-MG of group B3 were significantly higher than that of A,B1,B2group（ P〈 0. 05）; there was no significant difference between groups in the Scr level（ P〉 0. 05）. Conclusions Renal injury in elderly patients with COPD is closely related to the degree of hypoxia,which can be monitored by inspecting the degree of 1-MG,beta2-MG,Cys-C.
