摘要
目的观察凉血解毒利湿汤治疗急性湿疹(湿热浸淫证)的疗效及对血清IgE、肿瘤坏死因子-α(TNF-α)与白介素-4(IL-4)表达的影响。方法 80例患者按照随机数字表法分为治疗组和对照组各40例。对照组给予口服西替利嗪治疗,治疗组给予口服西替利嗪和凉血解毒利湿汤加减治疗。比较两组患者治疗后的临床疗效和体征、症状、舌象、脉象积分,血清Ig E、TNF-α和IL-4炎性细胞因子变化及不良反应。结果治疗组的总有效率92.50%优于对照组的72.50%(P<0.05)。治疗后两组体征、症状、舌象、脉象积分与治疗前比较均降低(均P<0.05),且治疗组优于对照组(均P<0.05)。治疗后,两组血清IgE、TNF-α、IL-4均低于治疗前(均P<0.05),且治疗组低于对照组(均P<0.05)。两组均出现大便稀、胃痛、口干、嗜睡、乏力等轻微不良反应,差别不大(P>0.05)。结论凉血解毒利湿汤治疗急性湿疹(湿热浸淫证)可减轻炎症反应,提高临床疗效,减少中医证候积分,不良反应轻。
Objective： To investigate the effect of Liangxue Jiedu Lishi Decoction on damp heat immersed type acute eczema and its influence on serum Ig E,TNF-a and IL-4 expression. Methods： 80 cases of damp heat immersed type acute eczema patients in June 2013 to January 2015 hospitalized were selected as the research objects. According to random number table method,they were divided into the treatment group and the control group,with 40 cases in each. The control group was given oral cetirizine,while the treatment group was given oral cetirizine and Liangxue Jiedu Lishi Decoction. The following data were compared： clinical curative effect and signs,symptoms,tongue and pulse condition integral between the two groups,and serum Ig E,TNF-α and IL-4 inflammatory cytokines were detected by enzyme linked immunosorbent assay（ELISA）,and the adverse reactions were observed. Results： The total efficiency in the treatment group was 92.50%,which was significantly higher than 72.50% of the control group;the difference was statistically significant（P〈0.05）. Before treatment,there were little difference in signs,symptoms,tongue and pulse condition integral between the two groups（P〈0.05）.After treatment,signs,symptoms,tongue and pulse condition integral of the two groups were lower than before treatment and the treatment group was significantly lower than that of the control group,the differences were statistically significant（P〈0.05）. The Ig E,TNF-α and IL-4 cytokine in the two groups of patients were lower than before treatment,and the treatment group was significantly lower than the control group;the stool,stomach pain,dry mouth,dizziness,fatigue and other adverse reactions in the two groups were mild,and the difference was not statistically significant（P〈0.05）. Conclusion： Liangxue Jiedu Lishi Decoction on damp heat immersed type of acute eczema can reduce the inflammatory response,improve clinical curative effect,and reduce the TCM syndrome score,with slight adverse reactions,worthy of clinical
