摘要
目的观察祛湿凉血方治疗慢性湿疹的临床疗效。方法将解放军总医院海南分院120例确诊为慢性湿疹的患者随机分为治疗组与对照组各60例,其中治疗组脱落2例,对照组脱落1例。治疗组给予口服祛湿凉血方(主要成分为龙胆草、炒栀子、生地黄、生薏仁等)1剂/d,2次/d;对照组给予复方氟米松软膏外涂2次/d,两组治疗疗程均为4周。实验结束后进行治疗前后皮损面积及严重度指数(eczema area and severity index,EASI)评分、瘙痒程度评分(VAS)并比较总有效率、复发率。结果治疗4周后中药组EASI评分及不良反应发生率均低于对照组[EASI:1.40(1.98)vs 1.60(2.00);不良反应:3.45%vs 10.17%],但差异无统计学意义(P均>0.05)。治疗组总有效率(87.9%)高于对照组(79.6%),但差异无统计学意义(P>0.05)。治疗组VAS评分及治疗后3个月复发率显著低于对照组[VAS:2.00(4.00)vs 5.00(5.00);复发率:10.3%vs25.4%],差异有统计学意义(P均<0.05)。结论祛湿凉血方能有效改善慢性湿疹EASI、VAS评分,具有较好临床疗效,且复发率较低、安全性较好。
Objective To observe the clinical effect of Qushi Liangxue Decoction in treatment of chronic eczema. Methods A total of 120 patients with chronic eczema were randomly divided into two groups： treatment group（n=60） and western medicine control group（n=60）. Two cases in treatment group and one case in control group dropped out. Patients in treatment group were treated with Qushi Liangxue Decoction for one dose twice a day and the control group was treated with compound ointment twice a day. Both two groups were treated for 4 weeks. EASI, VAS score, total effective rate and recurrence rate were compared before and after treatment. Results After 4 weeks of treatment, the EASI and side effect rate in the treatment group were lower than those in the control group with no significant difference [1.40（1.98） vs 1.60（2.00）, 3.45% vs 10.17%; P〉0.05, respectively]. The total effective rate in the treatment group was higher than that in the control group with no significant difference（87.9% vs 79.6%, P〈0.05）. The VAS score and the recurrence rate in the treatment group were significantly lower than those in the control group with significant difference [2.00（4.00） vs 5.00（5.00）, 10.3% vs 25.4%; P〉0.05, respectively]. Conclusion Qushi Liangxue Decoction is effective and safe in treating patients with chronic eczema, with less recurrence.
