摘要
目的探讨冠心病患者血清25(OH)D3水平及其与冠脉狭窄程度的相关性。方法选取经冠脉造影明确诊断的冠心病患者178例(冠心病组),冠脉造影明确排除冠心病者141例(非冠心病组)作为实验对象,冠心病组根据冠脉病变的支数,分为单支病变组、双支病变组、多支病变组,根据冠脉狭窄程度分为轻度狭窄组、重度狭窄组,全自动化生化检测仪检测生化指标,酶联免疫吸附试验检测25(OH)D3水平,Gensini积分了解冠脉狭窄程度。结果与对照组比较,实验组在收缩压、舒张压、血糖、低密度脂蛋白、脂蛋白a、Gensini积分等方面均明显升高,而25(OH)D3水平明显降低;冠脉多支病变组25(OH)D3水平较单支病变、双支病变组均明显降低;与冠脉轻度狭窄组比较,冠脉严重狭窄组患者血清25(OH)D3水平显著降低。进一步直线相关分析结果显示:冠心病患者血清25(OH)D3水平与冠脉Gensini积分、收缩压、舒张压、血糖均呈明显负相关(r分别为-0.814、-0.612、-0.574、-0.545,均P<0.01或P<0.01),而与低密度脂蛋白、脂蛋白a无明显相关性(均P>0.05)。结论冠心病患者血清25(OH)D3明显降低,其与冠脉狭窄程度呈明显负相关,对冠脉病变程度有一定的预测价值。
Objective To investigate the levels of serum 25- hydroxyvitamin D3 in patients with coronary atherosclerotic heart disease and its correlation with coronary arterial stenosis. Methods Totally 319 patients who underwent coronary angiograp Hy were divided into coronary heart disease group( n = 178) and the non- coronary heart disease group( n = 141) according to their coronary angiograp Hy. The patients in coronary heart disease group were divided into the single- vessel disease group,double- vessel disease group and the multi- vessel disease group. Meanwhile,they were divided into mild stenosis group and severe stenosis group according to their coronary Gensini score. The biochemical indicators were detected by automatic biochemical detector. The levels of serum 25- hydroxyvitamin D3 were detected by using Enzyme linked immunosorbent assay. The coronary arterial stenosis was evaluated by coronary Gensini score. Results Compared with non-coronary heart disease group,the indicators including systolic blood pressure,diastolic blood pressure,glucose,low density lipoprotein,lipoprotein a and Gensini score were significantly increased,but the levels of serum 25-hydroxyvitamin D3 were significantly decreased. The levels of serum 25- hydroxyvitamin D3 was significantly higher in the multi- vessel disease group than that of the single- vessel disease group and the double- vessel disease group. Compared with the mild stenosis group,the levels of serum 25- hydroxyvitamin D3 in the severe stenosis group was significantly decreased. Correlation analysis revealed that the levels of serum 25-hydroxyvitamin D3 in the patients with coronary heart disease was negatively correlated with Gensini score,systolic blood pressure,diastolic blood pressure and the glucose( r =- 0. 814,- 0. 612,- 0. 574 and- 0. 545 respectively,P 0. 05 or P 0. 01). Conclusions The levels of serum 25- hydroxyvitamin D3 in the patients with coronary heart disease are significantly decreased,and it is negatively correlated with the Gensini score and
