腹部内脏脂肪厚度与冠状动脉狭窄程度的关系

Correlation between visceral fat thickness and severity of coronary stenosis

摘要 目的探讨冠心病患者腹部内脏脂肪厚度(visceral fat thickness,VFT)与冠状动脉狭窄(coronarystenosis,CS)程度的关系。方法对173例冠心病患者CS程度应用Gensini积分进行评价,根据超声测量,分为低VFT组、中VFT组、高VFT 3组。结果与低VFT组比较,中VFT组、高VFT组患者腰围、体质量指数较大,甘油三酯明显增高,高密度脂蛋白胆固醇水平显著降低。随着VFT增加,Gensini积分明显增大。Gensini积分与年龄、腰围、体质量指数、甘油三酯、低密度脂蛋白胆固醇呈正相关(r=0.195~0.424,P【0.05),与高密度脂蛋白胆固醇呈负相关(r=-0.224,P=0.003)。校正年龄、性别、腰围后,VFT仍然与Gensini积分相关。结论CAD患者VFT与CS关系密切,VFT可能为CAD患者提供进一步的危险评估。 Objective To study the relationship between visceral fat thickness(VFT) and the severity of coronary stenosis(CS) in patients with coronary artery disease(CAD).Methods The study enrolled 173 patients with CAD whose CS severity was quantified in Gensini score,and VFT evaluated on ultrasonography.Patients were divided into low,medium,high value groups in accordance with VFT.Results Compared with the low VFT group,waist circumference(WC),body mass index(BMI) and triglycerides were higher in middle and high VFT group,while high-density lipoprotein cholesterol(HDL-C) was lower.The increase of VFT in the patients were associated with a high Gensini score.A significant positive correlation was demonstrated between Gensini score and age,WC,BMI,triglycerides,low-density lipoprotein cholesterol(r=0.195-0.424,P<0.05),the negative correlation of Gensini score with HDL-C(r=-0.224,P=0.003).VFT still associated with Gensini score after adjusting age,sex and WC.Conclusion VFT is significantly correlated with the severity of CS,which provides further risk assessment in patients with CAD.
