体重指数对老年心衰患者N末端脑钠肽浓度的影响

摘要 目的:探讨老年慢性心衰(CHF)患者N-末端脑钠肽(NT-proBNP)与体重指数(BMI)之间的关系及临床意义.方法:119例CHF患者按美国心脏病协会(NYHA)分级方法分为心功能Ⅱ级组(n=38)、Ⅲ级组(n=42)及心功能Ⅳ级组(n=38).以不同BMI值把每组再分为三亚组:体型正常(BMI〈24 kg/m^2)、超重(24 kg/m^2≤BMI<28kg/m^2)及肥胖亚1组(BMI≥28kg/m^2).ELISA法测定血浆Nt-proBNP浓度.比较不同心功能组Nt-proBNP浓度差异及同一组不同亚组间Nt-proBNP浓度差异,并在组内进行NT-proBNP与BMI相关性分析.结果:NT-proBNP随心功能分级程度升高而显著增加(P〈0.01).在同一心功能组中,正常、超重及肥胖亚组血浆NT-proBNP浓度逐渐下降,BMI与NT-proBNP之间存在负相关关系( P〈0.001).结论: 老年慢性心衰患者中血浆NT-proBNP浓度随BMI增加而降低.在应用NT-proBNP进行心功能评估时应考虑肥胖因素对其产生的影响. Objective: Discusses the old age chronic heart to fade (CHF) patient N- terminal brain sodium peptide (NT--proBNP) and between the body weight index (BMI) relations and the clinical significance. Methods: 119 example CHF patient divides into the heart function according to the American Heart disease Association (NYHA) graduation method Ⅱ Level group (n= 38), Ⅲ Level group (n= 42) and heart funetion Ⅳ Level group (n= 38). Divides into again by the different BMI value each group three subnits:Build normal (BMI(24 kg/m^2) ,overload (24 kg/m^2≤BMI (28 kg/m^2) and obese Asian 1 group (BMI≥28 kg/m^2). ELISA law determination blood plasma Nt-proBNP density. Compared with not the concentric function group Nt--proBNP density difference and the identical group during different subnit the Nt--proBNP density difference,and carries on NT--proBNP and the BMI relevance analysis in the group. But finally the NT--proBNP as one desires function graduation degree ascension obviously increases (P〈0. 01 ). In with wholeheartedly the function group in,normal,overwelght and the obese subnit blood plasma NT--proBNP density drops gradually,between BMI and NT--proBNP has the inverse correlation relations (P〈0. 001) Conclusion: The old age chronic heart fades in the patient the blood plasma NT--proBNP density to increase along with BMI reduces. When carries on the heart function appraisal using NT--proBNP should consider the obese factor the influence which produces to it.
