胸腰椎骨折后方韧带复合体损伤的MRI诊断 被引量:4

摘要 目的探讨胸腰椎骨折后方韧带复合体(PLC)损伤的MRI诊断方法与效果。方法选择2009年4月~2014年4月在我院骨科诊治的80例胸腰椎骨折患者,术前均经MRI检查,然后进行手术探查确诊。结果术前MRI诊断为PLC损伤28例,手术所见PLC损伤30例,MRI诊断的敏感性与特异性分别为93.3%和100.0%。随着PLC损伤程度的增加,脊髓损伤百分比明显增加(P〈0.05)。多因素Logistic回归分析显示,体重指数、腰椎间盘病变和骨赘为导致PLC损伤发生的主要独立危险因素(P〈0.05)。结论胸腰椎骨折中PLC损伤比较常见,体重指数、腰椎间盘病变和骨赘为主要的危险因素,MRI诊断PLC具有很好的敏感性、特异性。 Objective To investigate the MRI diagnosis methods and effects for posterior ligamentous complex(PLC) injury induced by the thoracic and lumbar fractures. Methods 80 thoracolumbar fracture patients treated between April 2009 and April2014 were selected. All the patients received preoperative MRI diagnosis and surgical exploration confirmation. Results There 28 cases diagnosed as PLC injury by MRI before the operation, while there were 30 cases of PLC injury discovered in the operation. The MRI diagnostic sensitivity and specificity were 93.3% and 100.0%. Along with the increase of PLC injury degree, the percentage of spinal cord injury significantly increased(P 0.05). Multivariate Logistic regression analysis showed that body mass index(BMI),waist disc disease, and osteophytes were the major independent risk factors of the PLC injury(P 0.05). Conclusion PLC injury is very common in thoracic and lumbar fractures. BMI, waist disc disease, and osteophytes are the main risk factors. MRI diagnosis has good sensitivity and specificity in the diagnosis of PLC.
