摘要
目的:构建以服务质量和效果为考核重点、基于乡村医生提供的基本医疗、基本公共卫生、慢性病管理、健康管理等整体服务的签约服务评价指标体系,拟将运用该指标体系得到的评价结果作为对开展签约服务乡村医生经费拨付的依据,从而起到对乡村医生的激励和约束作用。方法:通过文献研究和专家会商法确定评价指标体系的各项初始指标,运用两轮德尔菲法完成评价指标的选择及权重的确定,构建一套适用于乡村医生签约服务考核的评价指标体系。结果:构建的乡村医生签约服务评价指标体系共分为三级:一级指标有3个,二级指标有9个,三级指标有53个。结论:本研究构建的乡村医生签约服务评价指标体系,在促进乡村医生合理收入以及激励乡村医生工作积极性方面具有杠杆作用,且涵盖乡村医生提供的整体服务的考核省去了各分项考核的时间成本、并提高了评价的科学性。但是仍需对构建的指标体系进行进一步的实践论证。
Objective The aim is to construct the rural contract service evaluation index system whose key points are service quality and effect, and who based on the whole health service, then to regard the result of the contract service evaluation as the evidence of paying compensation funds to rural doctors who develop the contract service, thus to encourage and restrain the rural doc- tors. Methods Literature review and expert consultation were used to determine the original evaluation indicators. Then, completing the selection of evaluation index and the calculation of index weights by 2 rounds of Delphi, and then to construct the rural doctors evaluation index system. Results This study has constructed a rural contract service evaluation index system whose key points are service quality and effect. The index system included 3 fist-level indicators,9 second-level indication and 53 third-level indicators. Conclusions The rural doctor contract service evaluation index system constructed by this study has leverage effects on promoting the reasonable income of rural doctors and improving their work motivation. Besides, this index system has covered an assessment on the whole service offered by rural doctors ,which has decreased the time costs for various parts of assessments and has improved the scientificity of the evaluation. However,this constructed index system still needs to be further practically illustrated.
