摘要
目的分析术后泌尿系感染患儿病原菌分布及其药敏试验结果,为泌尿系感染的治疗提供参考依据。方法选取2013年1月-2015年6月430例术后发生泌尿系感染患儿,对送检的尿液检出病原菌采用API细菌鉴定系统鉴定到菌种,以K-B纸片法进行药敏试验,数据采用SPSS 19.0软件进行统计分析。结果 430例患儿送检尿液标本检出阳性122份,检出阳性率为28.37%;共培养出病原菌122株,检出阳性率为28.37%;以革兰阴性菌为主,共78株占63.93%;大肠埃希菌对亚胺培南敏感率为100.00%,肺炎克雷伯菌对亚胺培南、阿米卡星敏感率均为100.00%,铜绿假单胞菌对亚胺培南、阿米卡星、环丙沙星、左氧氟沙星、哌拉西林/他唑巴坦敏感率均为100.00%。结论以大肠埃希菌为主的革兰阴性菌是引起患儿术后发生泌尿系感染的主要病原菌,其次为肠球菌属,感染病原菌耐药现象普遍,应尽量根据药敏试验结果合理选择抗菌药物。
OBJECTIVE To analyze pathogen distribution and drug sensitive test for children with postoperative urinary tract infections,and to provide a reference for the treatment of urinary tract infections.METHODS A total of430 cases of children with postoperative urinary tract infections from Jan.2013 to Jun.2015 were selected.Urine was inspected and pathogens were tested by API bacterial identification systems.Drug sensitive test was conducted by K-B paper disk method.The results were statistically analyzed by software SPSS 19.0.RESULTS A total of 122 strains pathogens were detected from 430 samples of inspected urine,with a positive rate of 28.37%,specifically gram-negative bacteria for 63.93%,gram-positive bacteria for 32.78%,and fungi for 3.28%.For the constituent ratio of infection pathogens:E.coli accounted for 35.25%,Enterococcus for 25.41%,P.klebsiella bacteria for 10.66%,Pseudomonas aeruginosa for 8.20%;E.coli was sensitive to Imipenem at a rate of100.00%;K.pneumoniae was sensitive to imipenem and amikacin at the rate of 100.00%;P.aeruginosa was sensitive to amikacin,ciprofloxacin,levofloxacin,and piperacillin/tazobactam at the rate of 100.00%.CONCLUSIONE.coli-based gram-negative bacteria are the main pathogens causing children postoperative urinary tract infections,followed by Enterococcus.Pathogens cause drug resistance generally.It should be a reasonable choice of antibiotics based on the results of drug sensitive test.
