摘要
目的探讨Th1/Th2免疫平衡与胃癌前病变危险因素及病理积分的关系。方法采用酶联免疫法(ELISA)测定100例胃癌前病变患者血清Th1(IL-2、INF-γ)、Th2(IL-4、IL-6)含量,采集、检测患者年龄、性别、HP感染、遗传、饮食结构及病理积分,并逐一进行相关性分析。结果 HP感染患者细胞因子INF-γ、INF-γ/IL-4明显降低(P<0.05),IL-4明显升高(P<0.05),不良饮食结构患者IL-4的表达明显升高,INF-γ/IL-4明显降低(P均<0.05)。胃癌前病变胃镜病理积分与INF-γ/IL-4比值存在负相关关系。结论 HP感染、不良的饮食习惯等危险因素能使人体Th1/Th2免疫发生漂移,从而导致异常增生甚至癌变。
Objective To explore the relevance of the helper T（Th）1/Th2 cell balance and risk factors and pathological score in precancerous lesions of gastric cancer（PLGC）. Methods The values of Th1（IL-2, INF-γ）and Th2（IL-4, IL-6）in peripheral blood of 100 patients with PLGC were detected by ELISA. Age, gender, infection of helicobacter pylori（HP）, genetic background, dietary structure and gastral pathological score of patients with PLGC were collected. Then Th1/Th2 cytokine, the risk factors and pathological score in PLGC were processed by correlation analysis. Results The expression of INF- γ and the ratio of INF- γ/IL-4 decreased significantly（P〈0.05） and the expression of IL-4 increased obviously（P〈0.05）in the patients with HP infected. Meanwhile, the ratio of INF-γ/IL-4 decreased significantly（P〈0.05） and the expression of IL-4 increased obviously（P〈0.05） in the patients with unhealthy structure of diet. Gastral pathological score was negatively related to INF-γ/IL-4 in patients with PLGC. Conclusion Infection of HP, unhealthy structure of diet can influence Th1/Th2 balance, causing hyperplasia even canceration.
