摘要
目的探讨耐高压注射型双腔PICC导管（Power PICC）在静脉肿瘤患者中的应用效果及异位颈内静脉的调整方法。方法统计采用Power PICC的3258例肿瘤患者导管置管期间的并发症发生情况,并对其中26例异位颈内静脉患者进行导管调整,记录并比较导管在进入机体的10 min内、20~30 min这2个时间段的调整成功率。结果 26例Power PICC导管异位颈内静脉的肿瘤患者中,有8例患者在10 min内调整成功,调整成功率为30.8%,其余18例异位导管在25~30 min调整成功,调整成功率为100%。3258例置管患者中,发生渗液4.48%,渗血3.99%,皮疹9.97%,血栓0.64%,发生一侧堵管116次、两侧堵管42次。结论将Power PICC导管退回到异位起始处前1 cm,抬高头肩部30°,退出部分导丝,使导管在人体温度下软化25~30 min后再进行导管异位调整,能有效地将异位的导管调整置入上腔静脉下段。
Objective To explore application of high pressure resistant injected double cavity PICC catheter（ Power PICC） and adjustment of ectopic jugular vein. Methods The complications of 3258 tumor patients during PICC catheter were observed and 26 patients with ectopic jugular vein were adjusted. The success rate of adjustment in 10 min and 20 ~ 30 min after catheter was compared. Results Out of 26 tumor patients with ectopic jugular vein using power PICC catheter,8 adjusted successfully within 10 min,and success rate of adjustment was 30. 8%. The other 18 cases adjusted successfully in 25 ~ 30 min with success rate of adjustment of 100%. In all the 3258 patients with indwelling tube,the complications rates such as leakage,exudation,skin rash,thrombosis were4. 48%,3. 99%,9. 97% and 0. 64%. Blocking tube in one side occurred 116 times and 42 times for both tube blockage. Conclusion The measures including taking power PICC back to 1 cm at the beginning of the ectopic site,raising head and shoulder for 30°,withdrawing part of guide wire,softening the catheter for 25 ~ 30 min in the body temperature to adjust ectopic catheter can effectively adjust it to lower segment of superior vena cava.
