不同导尿时间及集尿袋对老年患者尿路感染的影响 被引量:7

摘要 目的探讨不同导尿时间及集尿袋对老年患者尿路感染的影响,为合理导尿提供依据。方法选择2014年6月~2015年6月本院收治的需要留置导尿的老年患者60例,随机分为A组和B组,A组采用单向活瓣集尿袋、B组采用普通无菌集尿袋,分别导尿第1、3、5、7、9天抽取尿液行病原菌培养,观察患者尿路感染情况。结果本组60例患者导尿第1、3、5、7、9天尿路感染逐渐增多,差异有统计学意义(P〈0.05);尿培养分离病原菌依次为大肠埃希菌、真菌、铜绿假单胞菌、金黄色葡萄球菌、克雷伯杆菌,其中大肠埃希菌感染率明显高于其他菌种(P〈0.05)。两组导尿3d尿路感染率无明显差异,A组患者导尿5、7、9天尿路感染率明显低于B组(P〈0.05);组内比较显示,A组3d与5、7d尿路感染率比较差异无统计学意义(P〉0.05),3d与9d尿路感染率差异显著(P〈0.05);B组3d与5d尿路感染差异无统计学意义(P〉0.05),3d与7、9d尿路感染差异有统计学意义(P〈0.05)。结论导尿时间、尿袋选择及更换时间是影响老年尿路感染的重要因素,导尿时间超过1周时患者尿路感染会明显增多,单向活瓣集尿袋可减少尿路感染,更换时间宜控制在每周1次,普通尿袋则应每周两次更换。 Objective To explore the effect of different catheterization time and urine collection bags on urinary tract infection of elderly patients and to provide references for rational catheterization. Methods 60 elderly patients requiring indwelling catheterization in our hospital from January 2014 to June 2015 were selected and they were randomly divided into group A and group B. Group A was received One-direction valve urine collection bags and group B was received ordinary sterile collection bags. Urine of the first, third, fifth, seventh, ninth day was pumped for pathogenic bacteria cultivation. Urinary tract infection of patients was observed. Results On first,third,fifth,seventh,ninth day of catheterization, urinary tract infection of 60 patients was gradually increased and the difference was statistically significant(P 〈 0.05).Separate pathogenic bacterium of urine cultivation was escherichia coli, fungi, pseudomonas aeruginosa, staphylococcus aureus and klebsiella. Infection rate of escherichia coli was significantly higher than that of other strains (P 〈 0.05). Rates of urinary tract infection of the third of catheterization in two groups had no significant difference.Rates of urinary tract infection of the fifth,seventh,ninth day of catheterization in the group A were significantly lower than group B(P 〈 0.05).Within group A, infection rates of third day and fifth and seventh day had no significant difference(P 〉 0.05),but it was significantly different from that of the ninth day(P 〈 0.05).Within group B, infection rates of third day and the fifth day had no significant difference(P 〉 0.05),but the difference in that of third day and seventh and ninth day was statistically significant (P 〈 0.05). Conclusion Catheterization time, urine bag selection and replacement time are important factors to urinary tract infection of elderly patients.Urinary tract infection will significantly increase when catheterization time is more than a week.One-direction valve collection bag
