摘要
为探讨吻合器痔上黏膜环切术（PPH）治疗环状混合痔的临床疗效，回顾分析应用PPH治疗的35例环状混合痔的临床资料。结果显示，35例均顺利完成手术。手术时间25～45min，平均30min。住院时间7～9d，平均8d。术中下腹部一过性疼痛5例，占14．3％；吻合口搏动性出血3例，占8．6％；术后出血1例，占2．9％；术后尿潴留3例，占8．6％；肛缘水肿2例；肛门疼痛4例，需用止痛药物缓解2例；术后排便困难2例；吻合口轻度狭窄2例。35例均一次性治愈，术后随访2～15个月，均无复发。结果表明，PPH治疗环状混合痔疗效确切，临床满意度高，具有术后并发症少、疼痛轻、恢复快等优点，但要注意病例选择、手术操作细节、并发症防治及与其他手术方式的配合运用。
In order to investigate the clinical effect of PPH for circumferential mixed hemorrhoids,authors retrospectively analyzed 35 patients＇ clinical data treated with PPH.As results,all of the 35 cases received surgery smoothly,the time for operation was 25-45 min （averaged 30 min）, hospitalization time was 7-9 days（averaged 8 days） ;intraoperative temporal hypogastric pain occured in 5 cases（accounted for 14.3 %）, stoma pulsating bleeding,in 3 cases （8.6%）,postoperative bleeding in one case（2.9%）;postoperative uros- chesis in 3 cases（8.6%）,ano-edge edema in 2 cases;anal pain in 4 cases（included 2 cases relieved by using stopping-pain drug）,postoperative dyschesia in 2 cases,light stenosis of stoma in 2 cases;the 35 cases were primarily cured, the postoperative follow-up of 2-15 months did not find any recurrence cases.Results show that PPH for circumferential mixed hemorrhoids has such advantages as certain effect,high clinical satis- factory degree,less postoperative complication,light pain and faster recovery, etc, but it should yet be no- ticed choosing cases, mastering operative skill, preventing ＆ treating complication, and coordinating it with other procedures.
