摘要
[目的]探讨血清胃蛋白酶原Ⅰ(Pepsinogen,PGⅠ)、胃泌素(Gastrin,GS)与幽门螺杆菌相关性消化性溃疡的关系。[方法]收集2013年4月~2014年5月来我院消化科就诊幽门螺杆菌感染阳性的消化性溃疡患者(Hp+-PU组)69例,Hp感染阴性的消化性溃疡患者51例(Hp--PU组)以及41例因胃部需作为胃镜检查以及病理组织活检人员作为对照组,采用酶联免疫法加测血清中血清PGⅠ、GS,免疫组化检测胃部组织黏膜PGⅠ、GS表达情况,logistic回归分析PGⅠ、GS与幽门螺杆菌相关性消化性溃疡的相关性。[结果]ELISA检测结果显示,Hp--PU组和Hp+-PU组患者血清中的GS的水平显著高于对照组,差异有统计学意义(P〈0.01);Hp+-PU组患者血清中的GS水平为(100.28±13.77)pg/ml,高于Hp--PU组的(69.04±9.21)pg/ml,差异有统计学意义(P〈0.01);Hp+-PU组PGⅠ水平高于对照组、Hp--PU组,差异均有统计学意义(P〈0.05);Hp+-PU患者十二指肠溃疡组GS含量明显高于胃溃疡组,差异有统计学意义(P〈0.05);免疫组化结果显示,Hp--PU组和Hp+-PU组患者的胃黏膜组织GS阳性细胞百分比分别为(9.89±1.19)%、(33.57±4.29)%,与对照组(1.65±0.39)%比较差异有统计学意义(P〈0.05或P〈0.01);Hp--PU组与Hp+-PU组比较,差异有统计学意义(P〈0.01)。对照组中PGⅠ阳性细胞占(9.33±1.31)%,低于Hp--PU组和Hp+-PU组的(25.08±3.45)%和(59.88±6.43)%,且差异有统计学意义(P〈0.05或P〈0.01);logistic分析结果显示,GS、Ps与幽门螺杆菌阳性消化性溃疡呈正相关。高GS可能是幽门螺杆菌阳性消化性溃疡的独立危险因素[OR(95%CI)为1.132(1.073~1.194)]。[结论]血清中GS、Ps水平的升高与Hp感染消化性溃疡有相关性。
[Objective]To investigate the relationship between serum pepsinogen-Ⅰ(PGⅠ),gastrin and Helicobacter pylori-associated peptic ulcer.[Methods]From April 2013 to May 2014,sixty-nine peptic ulcer patients with Helicobacter pylori infection(Hp+-PU group)in our hospital,51 cases of peptic ulcer patients with Hp-negative(Hp--PU group)and 41 persons needing gastric endoscopy and biopsy pathology inspection as a control group.Enzyme-linked immunosorbent assay was used to measure sera PGⅠ,gastrin.Immunohistochemistry was applied to test PGⅠand gastrin expression in gastric mucosal tissue.Logistic regression analysis was applied to analyse the correlation between PGⅠ,gastrin and Hp-associated peptic ulcer.[Results]Levels of serum GS in the Hp--PU group and Hp+-PU group were significantly higher than the control with significant difference(P〈0.01).Serum GS in the Hp+-PU group was 100.28±13.77pg/ml,higher than that in the Hp--PU group 69.04±9.21pg/ml,with a statistically significant difference(P〈0.01);PGⅠlevel in the Hp+-PU group was higher than that in the Hp--PU group,with statisticallysignificant difference(P〈0.05).Percentage of GS positive cells in the Hp+-PU patients and the Hp--PU group was 9.89±1.19 % and 33.57±4.29%,which was significantly higher than the control group 1.65±0.39%(P〈0.05 or P〈0.01).Hp+-PU group was significantly higher than Hp--PU group(P〈0.01).PGⅠ positive cells in the control group accounted for 9.33±1.31 %,lower than the Hp--PU group and Hp+-PU group(25.08±3.45)% and(59.88±6.43)%,and the difference was statistically significant(P〈0.05 or P〈0.01).Logistic analysis showed that GS,Ps and Helicobacter pylori-positive peptic ulcer were positively correlated.High GS may be an independent risk factor for H.pylori-positive peptic ulcer[OR(95% CI)of 1.132(1.073~1.194)].[Conclusion]The elevated levels of serum GS,PGⅠwere collelated with peptic ulcer infected with H.pylori.
