摘要
围绝经期抑郁症多指女性在围绝经期发生的一种特殊的精神障碍。病因病机极其复杂,呈立体、多层次状,涉及阴阳、虚实、五脏六腑、气血精液。由女子"七七"、肾气衰所造成的肾阳虚衰是导致围绝经期抑郁症一系列证型的原始病机,在此基础上,由肾阳虚导致肾精不足、肾阴亏虚、阴阳失衡,进而累及五脏六腑、气血精液,又是直接造成各种临床证型的继发病机。本文通过阐述围绝经期人体脏腑气血的变化,对围绝经期抑郁症病机进行层层剖析,探讨中医病机和现代医学研究之间的相关性,指出中医辨证和治疗要依据原始病机,也要重视直接造成各种临床证型的继发病机,强调在固护肾阳的基础上,协调五脏、通调气机、彰明神志。
Perimenopausal Depressive Disorder is a special kind of mental disorder of Perimenopausal women.Its etiology and pathogenesis,according to the theory of Traditional Chinese Medicine,is extremely complex.It relates to Yin and Yang,Deficiency and Excess,the Five Internal Organs and The Six Hollow Organs,Qi and Blood,Nutrient Substance and Body Fluid.Kidney Qi deficiency and imbalance of yin and yang could cause kidney yang deficiency.Since kidney yang deficiency plays the most important role in the pathogenesis of Perimenopausal Depressive Disorder,it becomes the basic pathogenesis of Perimenopausal Depressive Disorder.Furthermore,kidney yang deficiency could cause viscera disorder.The secondary pathogenesis that caused by viscera disorder becomes the direct cause of all Perimenopausal Depressive Disorder clinic symptoms.This paper,through the analysis of the particularity of the pathogenesis of Perimenopausal Depressive Disorder in both Chinese Medicine and Western Medicine,introduces the changes of Qi and Blood in viscera of human menopausal transition;and points out that syndrome differentiation of Traditional Chinese Medicine should not only to pay attention to the basic pathogenesis of Perimenopausal Depressive Disorder,but also should to pay attention to those secondary pathogenesis of Perimenopausal Depressive Disorder that directly cause a variety of clinical syndromes.The study emphasizes on protecting kidney yang,coordinating viscera and making Qi and blood in harmony.Because of this effort,the human body Qi activity would be unobstructed and human consciousness would be clear.
