摘要
目的:探讨B型钠尿肽(BNP)在舒张性心功能不全(DHF)中的诊断意义。方法:选取我院2014年9月~2015年9月收治的50例DHF患者作为DHF组,另选取30例同期住院非心功能不全患者为对照组,采用彩色多普勒超声诊断仪测量左室舒张末期内径(LEVDD)、左房内径(LAD)、左心射血分数(LVEF),四腔切面脉冲多普勒测量二尖瓣舒张早期血流充盈速度峰值(E)及二尖瓣舒张早期峰值运动速度(Ea),取平均值计算E/Ea值;采用全自动电化学发光免疫分析系统测定血浆BNP水平。结果:随着心功能分级的加重,LAD、LEVDD、E、E/Ea值逐渐升高,LVEF、Ea值逐渐下降,与对照组比较,差异有统计学意义(P〈0.05);DHF组BNP水平明显高于对照组,差异有统计学意义(P〈0.05);随着NYHA分级的增加,BNP水平呈逐渐上升趋势,各级间比较差异有统计学意义(P〈0.05)。结论:BNP水平随NYHA心功能级别增加而升高,对DHF的早期诊断、危害判断有重要意义,其测定简便易行,准确度高,值得临床推广。
Objective: To explore the significance of B-type natriuretic peptide (BNP) in the diagnosis of diastolic heart failure (DHF). Methods: A total of 50 patients with DHF who were admitted in our hospital were included in the study and served as DHF group. Moreover, 30 patients without heart failure were served as the control group. Color Doppler ultrasonic diagnostic apparatus was used to measure LEVDD, LAD, and LVEF. The four-chamber-view pulse Doppler was used to detect E and Ea. The average value was taken and E/Ea was calculated. The full automatic electrochemilumineseence immunoassay was used to determine the plasma BNP level. Results: With the aggravation of cardiac function grading, LAD, LEVDD, E, and E/Ea were significantly increased, but LVEF and Ea were significantly reduced when compared with the control group (P0.05). BNP level in DHF group was significantly higher than that in the control group (P〈0.05). With the increase of NYHA grading, BNP level was gradually increased, and there were significant differences between the two groups (P〈0.05). Conclusions: BNP level is increasing with the increase of NYHA grading, which is of great significance in the early diagnosis of DHF, and the hazard estimation, and it is characterized by simple operation and high accuracy; therefore, it is widely recom- mended in the clinic.
