摘要
[目的]探讨膝骨性关节炎(osteoarthritis,OA)膝关节液内微量元素的浓度与Outerbridge分级标准的关系。[方法]选择2012年1月~2014年1月本科确诊的膝关节OA患者160例,根据骨性关节炎关节镜下Outerbridge分级分4组,以40例健康体检者为对照,抽取各组膝关节液,检查关节液中铜、钙、镁、锌、铁元素的浓度。[结果]对照组与膝关节OA组关节液的铜、钙、镁、锌和铁浓度均有显著差异(P〈0.01)。不同Outerbridge的膝关节OA组内关节液的铜、钙、镁、锌和铁浓度均有显著差异(P〈0.01)。与Outerbridge分级相关的因素有关节液的钙、锌、铁和铜,其中铁为负相关(P〈0.05或P〈0.01),与镁无相关性。[结论]膝关节液中钙、锌、铁和铜的浓度变化可作为膝关节OA的诊断指标,且钙、锌、铁和铜与Outerbridge分级标准有相关性,可用作提示膝关节OA病变严重程度的指标。
[ Objective] To investigate the relationship of elements in synovial fluid with Outerbridge scale of knee osteoar- thritis (OA) patients. [ Methods ] One hundred and forty OA patients were randomly divided into four subgroups in terms of Outerbridge scale I, II, III, IIII, with 40 patients in each group. Another 40 healthy volunteers were enrolled as the control group. Cu. Zn. Ca. Mg and Fe in synovial fluid of all subjects were determined by USA VARIAN Vista - MPX. The differences were compared between experiment subgroups and control group. [ Results] Compared with the control group, Cu. Zn. Ca. and Mg increased, while Fe decreased in the synovial fluid of OA group, with significantly statistical difference (P 〈 0. 01 ). With- in 40A subgroups, Cu. Zn. Ca Mg and Fe were different ( P 〈 0. 01 ), indicating that the contrentration of them was related to the different Outerbridge scales . Logistic analysis showed that Cu. Zn. Ca. were positive, Fe was negative ( P 〈 0. 01 or P 〈 0. 05), and Mg had no relationship with Outerbridge scale of knee OA. ~ Conclusion ] Cu. Zn. Ca and Fe in synovial fluid have diagnostic value for defining knee OA. The correlation of Cu. Zn. Ca and Fe in synovial fluid can indicate the severity of knee OA.
