降钙素原与C-反应蛋白检测细菌性感染的应用研究 被引量:33

摘要 目的探讨降钙素原(PCT)与C-反应蛋白(CRP)检测细菌性感染的应用价值,为临床诊治提供参考。方法选取2014年11月-2015年7月医院收治的确诊为细菌感染患者97例,将其分为重症细菌感染组(A组)47例和局部细菌感染组(B组)50例,选取同期病毒感染患者46例设为病毒感染组(C组),同时选取47名健康者自愿参与试验作为对照组(D组);对比4组患者PCT、CRP水平、A组与C组的PCT、CRP的诊断效率、治疗前后重症细菌感染患者的PCT、CRP的水平比较,采用SPSS18.0软件进行统计分析。结果 A组的PCT显著高于B、C、D 3组,CRP显著高于D组(P<0.05),与B、C组差异无统计学意义;B组的PCT显著高于C、D两组,CRP显著高于D组(P<0.05),与B、C组差异无统计学意义;C组的PCT和CRP均高于D组(P<0.05);以PCT>0.5μg/L的标准为界,PCT诊断细菌性感染的敏感度为95.74%,特异性为95.65%;以CRP>8mg/L的标准为界,CRP诊断细菌性感染的敏感度达95.74%,特异性为91.30%;经治疗后,A组患者的PCT、CRP的水平显著降低,差异有统计学意义(P<0.05)。结论 PCT联合CRP检测细菌性感染效果显著,同时可以依据PCT和CRP的检测结果评估对细菌感染患者的治疗效果,值得在临床上推广应用。 OBJECTIVE To explore the value of procalcitonin(PCT)and C-reactive protein(CRP)in detection of bacterial infections so as to provide guidance for clinical diagnosis and treatment.METHODS A total of 97 patients with confirmed bacterial infections who were treated in the hospital from Nov 2014 to Jul 2015 were enrolled in the study and divided into the severe bacterial infection group(group A)with 47 cases and the local bacterial infection group(group B)with 50 cases,meanwhile,46 patients with viral infections were chosen as the viral infection group(the group C),and 47 healthy people who volunteered to participate were assigned as the control group(group D).The levels of PCT and CRP were compared among the four groups of patients,the effective rates of PCT and CRP in the diagnosis were compared between the group A and the group C,and the levels of PCT and CRP of the patients with severe bacterial infections were compared before and after the treatment.The statistical analysis was performed with the use of SPSS 18.0software.RESULTS The PCT level of the group A was significantly higher than that of the group B,C,and D;the CRP level of the group A was significantly higher than that of the group D,as compared with the group B and C,the difference was not significant.The PCT level of the group B was significantly higher than that of the group C and D,and the CRP level of the group B was significantly higher than that of the group D(P〈0.05),as compared with the group B and C.The levels of PCT and CRP of the group C were higher than those of the group D(P〈0.05).By setting the PCT level of more than 0.5μg/L as the standard,the sensitivity of the PCT in diagnosis of the bacterial infections was 95.74%,the specificity95.65%.By setting the CRP level of more than 8mg/L as the standard,the sensitivity of the CRP in diagnosis of the bacterial infections was 95.74%,the specificity 91.30%.The levels of both PCT and CRP of the group A were significantly reduced after the treatment(P〈0.05)
