摘要
目的了解老年下呼吸道感染患者病原菌分布及其对抗菌药物的耐药性分析,为临床合理使用抗菌药物提供帮助。方法回顾性分析2010年1月-2013年12月老年下呼吸道感染患者痰液5 768份标本分离出病原菌分布及药敏结果,数据采用WHONET 5.6软件进行统计分析。结果共5 768份痰标本,其中1 598份检出病原菌,总检出率27.7%;共检出病原菌1 626株,以肺炎克雷伯菌、鲍氏不动杆菌、金黄色葡萄球菌为主,分别占40.7%、24.8%、16.0%;鲍氏不动杆菌对氨曲南、磺胺甲噁唑/甲氧苄啶、哌拉西林/他唑巴坦、头孢唑林、头孢噻肟耐药率较高,均>75.0%。结论肺部感染以革兰阴性菌为主,主要包括肺炎克雷伯菌和鲍氏不动杆菌,临床医师应根据细菌鉴定和药敏结果合理使用抗菌药物,以便提高疗效和减少耐药菌株的发生。
OBJECTIVE To research the composition of pathogenic microorganisms in elderly patients with lower respiratory tract infections and its drug resistance to antimierobial agents, so as to provide help for rational clinical use of antibiotics. METHODS A retrospective analysis of sputum was conducted to identify pathogenic bacteria and drug sensitivity in elderly patients with lower respiratory tract infections from Jan. 2010 to Dec. 2013 in our hospital. WHONET 5.6 was explored to make statistical analysis. RESULTS During the 4 years in our hospital, the microbiology laboratory received a total of 5768 sputum specimens from elderly patients with lower respiratory tract infections. Among them, 1598 copies were found to contain pathogenic bacteria, and the total detection rate was 27.7%. Totally 1626 strains of pathogens were detected, in which mostly were Klebsiella pneumoniae, which accounted for 40.7%; Acinetobacter baumannii were the second, accounting for 24.8%, Staphylococcus aureus were the third, accounting for 16.0%. A. baumannil were found to be highly resistant to aztreonam, sulfamethoxazole/trimethoprim, piperacillin/tazobactam, cefazolin and cefotaxime and the drug resistance rates were all over 75.0%. CONCLUSION Pulmonary infection was mainly caused by gram-negative bacteria, including K. pneumoniae and A. baumannii. Clinicians should hase on the identification of bacteria and drug sensitivity results to rational use antimicrobial agents, so as to improve the curative effect and reduce the occurrence of drug resistant strams.
