摘要
目的：探讨微量白蛋白（mAlb）检验在糖尿病肾病中的诊断价值。方法选取2012年8月~2015年1月我院收治的糖尿病肾病患者74例为观察组,并选取同时期健康体检者74例为对照组,对比两组的mAlb含量和mAlb阳性情况,以及不同病程下的mAlb含量和mAlb阳性情况。结果观察组mAlb含量、mAlb阳性率分别为（53.26＆#177;4.75）mg/L、60.8%,显著高于对照组的（11.30±1.32）mg/L、0.0%（t=73.216,P〈0.01；x^2=64.660,P〈0.01）。病程〉13年者mAlb含量、mAlb阳性率分别为（74.36±5.65）mg/L、93.8%,均高于病程在1.2~6年的（24.61±3.08）mg/L、28.6%（t=30.431,P〈0.01；x^2=7.31,P〈0.01）,以及病程在7~12年的（34.16±4.17）mg/L、59.1%（t=29.944,P〈0.05；x^2=4.082,P〈0.05）。病程在7~12年者mAlb含量、mAlb阳性率高于病程在1.2~6年者（t=7.891,P〈0.01；x^2=4.933,P〈0.05）。结论mAlb在糖尿病肾病诊断中具有重大意义,可以为糖尿病肾病的诊断、病程监测、预后效果等提供参考性数据。
Objective To investigate the diagnostic value of microalbuminuria in diabetic nephropathy. Methods 74 patients with diabetic nephropathy in our hospital from August 2012 to January 2015 were selected as the observation group,while 74 cases with the healthy physical examination were selected as control group.The content of mAlb and the positive rate of mAlb in two groups were compared,and the content of mAlb and the positive rate of mAlb in different course were compared. Results The content of mAlb and the positive rate of mAlb in observation group was （53.26±4.75） mg/L,60.8% which was higher than the control group of （11.30±1.32） mg/L,0.0% （t=73.216,P〈0.01;χ2=64.660,P〈0.01）.The content of mAlb and the positive rate of mAlb in patients whose course was more than 13 years were （74.36±5.65） mg/L,93.8%,which were higher than those of the course of （24.61±3.08） mg/L and 28.6% for 1.2-6 years （t=30.431,P〈0.01;χ2=7.31,P〈0.01） and （34.16±4.17） mg/L and 59.1% for 7-12 years （t=29.944,P〈0.05;χ2=4.082,P〈0.05）. The content of mAlb and the positive rate of mAlb in patients with the course of 7-12 years were higher those of the course of 1.2-6 years （t=7.891,P〈0.01;χ2=4.933,P〈0.05）. Conclusion Microalbuminuria in the diagnosis of diabetic nephropathy is of great significance,and can provide the reference data for diabetic nephropathy diagnosis,disease course monitoring and prognosis effect.
