摘要
目的比较单切口与双切口超声乳化白内障吸除、折叠式人工晶状体植入联合青光眼小梁切除术治疗青光眼并发白内障的临床疗效。方法选取2013年2月—2014年6月九江学院附属医院收治的青光眼并发白内障患者46例,随机分为对照组与观察组,各23例。对照组患者予以单切口超声乳化白内障吸除、折叠式人工晶状体植入联合青光眼小梁切除术治疗,观察组患者予以双切口超声乳化白内障吸除、折叠式人工晶状体植入联合青光眼小梁切除术治疗。观察两组患者手术前后眼压、最佳矫正视力、裸眼视力情况及滤过泡(功能型滤过泡、包裹型滤过泡、缺如型滤过泡、弥漫扁平型滤过泡、微小型滤过泡)形成情况。结果术前两组患者眼压、最佳矫正视力、裸眼视力比较,差异无统计学意义(P>0.05);术后观察组患者眼压低于对照组,最佳矫正视力、裸眼视力高于对照组,差异有统计学意义(P<0.05);观察组患者功能型滤过泡形成率高于对照组,差异有统计学意义(P<0.05),两组患者包裹型滤过泡、缺如型滤过泡、弥漫扁平型滤过泡、微小型滤过泡形成率比较,差异无统计学意义(P>0.05)。结论双切口超声乳化白内障吸除、折叠式人工晶状体植入联合青光眼小梁切除术治疗青光眼并发白内障的临床疗效优于单切口超声乳化白内障吸除、折叠式人工晶状体植入联合青光眼小梁切除术,双切口超声乳化白内障吸除、折叠式人工晶状体植入联合青光眼小梁切除术可降低患者眼压,改善患者视力。
Objective To compare the effect between single incision and double incision phacoemulsification,fold-able intraocular lens implantation combined with glaucoma trabeculectomy in treating glaucoma and cataract. Methods A total of 46 patients with glaucoma and cataract were selected in Affiliated Hospital of Jiujiang University from February 2013 to June 2014,they were randomly divided into control group and observation group,23 cases in each group. Control group were given single incision phacoemulsification,foldable intraocular lens implantation combined with glaucoma trabeculectomy treatment, observation group were given double incision phacoemulsification,foldable intraocular lens implantation combined with glaucoma trabeculectomy treatment. Before and after operation,intraocular pressure,best corrected visual acuity,naked visual and forma-tional situation of filtering blebs（functional filtering bleb,encapsulated type filtering bleb,lack of such as type filtering bleb, diffuse flat type filtering bleb,microminiature filtering bleb）. Results Before operation,intraocular pressure,best corrected visual acuity,naked visual showed no significant differences between the two groups（P ﹥ 0. 05）;after operation,intraocular pressure of observation group was lower than that of control group,best corrected visual acuity and naked visual of observation group were higher than those of control group（P ﹤ 0. 05）;functional filtering bleb formation rate of observation group was higher than that of control group（P ﹤ 0. 05）,encapsulated type filtering bleb,lack of such as type filtering bleb,diffuse flat type fil-tering bleb,microminiature filtering bleb formation rate showed no significant differences between the two groups（P ﹥ 0. 05）. Conclusion Double incision phacoemulsification,foldable intraocular lens implantation combined with glaucoma trabeculectomy in treating glaucoma and cataract has better clinical effect compare with single incision phacoemulsification,foldable intraocular lens implantation
