血管内皮生长因子和骨形态发生蛋白2在非创伤性股骨头坏死不同区域的表达 被引量:12

摘要 目的:探讨血管内皮生长因子(vascular endothelial growth factor,VEGF)和骨形态发生蛋白2(bone morphogenetic protein 2,BMP-2)在非创伤性股骨头坏死不同区域的表达情况。方法:将从单侧人工髋关节置换术中取得的40个非创伤性坏死股骨头沿冠状面自正中用摆锯剖开,分别在坏死区、交界区和正常区各凿取1块约1 cm×1 cm×0.4 cm的骨块,制成石蜡切片,采用免疫组织化学法测定不同区域VEGF和BMP-2的表达情况。结果:1VEGF表达结果。3个区域VEGF阳性面积百分比比较,差异有统计学意义[(6.25±2.61)%,(19.30±4.56)%,(11.63±4.12)%;χ2=81.100,P=0.000];坏死区阳性面积百分比小于交界区和正常区(χ2=81.040,P=0.000;χ2=22.200,P=0.000),交界区大于正常区(χ2=18.410,P=0.000)。3个区域VEGF表达平均灰度比较,差异有统计学意义(86.30±6.84,31.85±4.89,73.93±5.78;F=939.984,P=0.000);坏死区平均灰度大于交界区和正常区(P=0.000;P=0.000),交界区小于正常区(P=0.000)。2BMP-2的表达结果。3个区域的BMP-2阳性面积百分比比较,差异有统计学意义[(5.20±2.46)%,(17.43±4.77)%,(10.03±4.32)%;χ2=76.450,P=0.000];坏死区阳性面积百分比小于交界区和正常区(χ2=76.320,P=0.000;χ2=21.950,P=0.000),交界区大于正常区(χ2=27.400,P=0.000)。3个区域BMP-2表达平均灰度比较,差异有统计学意义(88.65±6.61,41.25±5.00,76.80±5.33;F=751.985,P=0.000);坏死区BMP-2表达平均灰度大于交界区和正常区(P=0.000;P=0.000),交界区小于正常区(P=0.000)。结论:VEGF和BMP-2在非创伤性股骨头坏死交界区表达强度高于坏死区和正常区,交界区可能是影响股骨头坏死预后的关键区域。 Objective:To explore the expressions of vascular endothelial growth factor(VEGF)and bone morphogenetic protein 2 (BMP -2)in different zones of femoral head with non-traumatic osteonecrosis.Methods:Forty non-traumatic necrotic femoral head achieved via unilateral artificial hip replacement were splited along the median coronal plane by using swing saw,and a 1 .0 ×1 .0 ×0.4 -cm specimen was fetched from the necrotic zone,junctional zone and normal zone respectively and was made into paraffin sections.Then the expressions of VEGF and BMP -2 were detected by using immunohistochemical method.Results:There was statistical difference in the pos-itive area percentage of VEGF between the 3 zones(6.25 +/-2.61 ,1 9.30 +/-4.56,1 1 .63 +/-4.1 2%;χ2 =81 .1 00,P =0.000),and the necrotic zone was inferior to the junctional zone and normal zone(χ2 =81 .040,P =0.000;χ2 =22.200,P =0.000)while the junctional zone surpassed the normal zone(χ2 =1 8.41 0,P =0.000).There was statistical difference in the average gray value of VEGF expression be-tween the 3 zones(86.30 +/-6.84,31 .85 +/-4.89,73.93 +/-5.78;F =939.984,P =0.000),and the necrotic zone surpassed the junc-tional zone and normal zone(P =0.000;P =0.000)while the junctional zone was inferior to the normal zone(P =0.000).There was statis-tical difference in the positive area percentage of BMP -2 between the 3 zones(5.20 +/-2.46,1 7.43 +/-4.77,1 0.03 +/-4.32%;χ2 =76.450,P =0.000),and the necrotic zone was inferior to the junctional zone and normal zone(χ2 =76.320,P =0.000;χ2 =21 .950,P =0.000)while the junctional zone surpassed the normal zone(χ2 =27.400,P =0.000).There was statistical difference in the average gray value of BMP -2 expression between the 3 zones(88.65 +/-6.61 ,41 .25 +/-5.00,76.80 +/-5.33;F =751 .985,P =0.000),and the necrotic zone surpassed the junctional zone and normal zone(P =0.000;P =0.000)while the junctional zone was inferior to the normal zone.Conclusion�
