全膝置换术后早期两种康复训练方法的效果比较 被引量:12

Comparison between two early rehabilitation methods after total knee arthroplasty surgery

摘要 目的:观察两种康复训练方法在全膝关节置换术后早期患者中的应用效果。方法将60例人工全膝关节置换术后患者,采用随机数字表法分为对照组和观察组,每组各30例。对照组采用常规康复训练法,观察组患者利用双上肢借助绷带进行下肢主动和被动锻炼。术后1周、1,3个月对两组患者进行美国特种外科医院膝关节评分( HSS);术后1周进行膝关节活动范围测定,术后3 d行静息疼痛评分。结果术后1周、1,3个月观察组患者的膝关节 HSS评分分别为(63.90±2.10),(71.65±2.50),(73.80±2.80)分,均优于对照组,差异有统计学意义(t值分别为6.782,4.547,9.128;P〈0.01)。术后1周观察组患者的膝关节活动范围为(109.20±2.33)°,优于对照组,差异有统计学意义(t =16.644,P 〈0.01)。术后3 d 观察组患者额的静息疼痛评分低于对照组,差异有统计学意义( t =-4.613,P〈0.01)。结论膝关节置换术后早期采用患者利用双上肢借助绷带进行下肢主动和被动锻炼的方法,有利于加速膝关节功能的恢复,提高患者的生活质量,更好的巩固手术治疗效果。 Objective To compare the effects of two early rehabilitation methods after total knee arthroplasty surgery. Methods A total of 40 patients after total knee arthroplasty surgery were randomly assigned to control group and experimental group on average. The control group conducted the conventional rehabilitation method,and the experimental group performed a method that patients used upper limbs doing active motion and passive motion of lower limbs with bandage. The patients were assessed by Hospital for Special Surgery ( HSS) Knee Score at 1 week, 1 month and 3 months after the operation. The range of motion at 1 week and pain at 3 days were evaluated. Results At 1 week, 1 month and 3 months follow-ups, the HSS scores in the experimental group were (63. 90 ± 2. 10), (71. 65 ± 2. 50), (73. 80 ± 2. 80) points, significant better than those in the control group (t=6. 782, 4. 547, 9. 128;P〈0. 01). The rang of motion at 1 week and pain during rest at 3days follow-up in the experimental group were significantly better than those in the control group ( t=16. 644,-4. 613;P〈0. 01). Conclusions The new rehabilitation method can help to accelerate the recovery of knee function, improve the quality of life and consolidate the clinical outcomes.
