摘要
目的 研究动力髋螺钉(DHS)与股骨近端髓内钉(PFN)治疗老年股骨转子间骨折的临床效果研究.方法 将襄汾县人民医院90例老年股骨转子间骨折患者随机分为PFN组与DHS组,每组45例,比较两组手术指标(手术时间、术中出血量、住院时间及切口长度)与治疗效果.结果 PFN组、DHS组切口长度分别为(13.67±1.87)、(23.17±3.20)em,手术时间分别为(53.11±16.10)、(88.10±22.97) min,术中失血量分别为(279.54±118.31)、(476.30±174.61) ml,住院时间分别为(20.61±5.21)、(24.60±5.48) min,PFN组均显著低于DHS组,差异均有统计学意义(P<0.05);PFH组优良率(73.33%)显著高于DHS组(46.67%),差异有统计学意义(P<0.05).结论 PFN内固定术较为适合老年转子间骨折患者的骨质特点,同时具有手术创伤小、康复快、手术时间及手术失血量低的优势,可作为老年股骨转子间骨折患者治疗的优选内固定方案.
Objective To study the clinical application of dynamic hip screw (DHS) and proximal femoral intramedullary nail (PFN) in the treatment of senile femoral fractures between rotor.Methods Ninety patients with senile femoral fractures in people' s hospital of Xiangfen were randomly divided into PFN group and DHS group,with 45 cases in each group.The operation indicators (operation time,intraoperative blood loss,hospitalization time and incision length) and the effects were compared between the two groups.Results In PFN group and DHS group,the incision length were respectively (13.67 ± 1.87) cm and (23.17 ±3.20) cm,the operation time were respectively(53.11 ± 16.10)rain and (88.10 ±22.97) min,the intraoperative blood loss were respectively (279.54 ± 118.31) ml and (476.30 ± 174.61) ml,the length of hospital stay were respectively (20.61 ± 5.21)rmin and (24.60 ± 5.48) min,they were all significantly lower than that in DHS group,the differences were significant (P 〈 0.05).The excellent of PFH group was 73.33%,which was significantly higher than that of DHS group (46.67%),the difference was significant (P 〈0.05).Conclusions PFN fixation is more suitable for the elderly between the rotor characteristics of bone fracture patients,at the same time has a small surgical trauna,quicker recovery and with the advantage of short operation time and low blood loss,so it can be used as treatment in elderly patients with femoral fractures between rotor scheme of optimizing internal fixation.
