摘要
目的探讨吡格列酮联合克罗米芬治疗多囊卵巢综合征对胰岛素、性激素水平及排卵率的影响。方法选取2011年8月—2014年4月在我院妇产科就诊的多囊卵巢综合征患者74例,应用随机数字表法,将其分为试验组和对照组,每组各37例。对照组给予克罗米芬治疗,试验组给予吡格列酮联合克罗米芬治疗,两组均治疗3个月。观察并比较两组患者治疗前后空腹胰岛素(FINS)、促黄体生成素(LH)、促卵泡刺激素(FSH)及睾酮(T)水平,并计算胰岛素抵抗水平(Homal R)和胰岛素分泌曲线下面积(AUC);然后比较两组患者排卵率及受孕率的差别。结果两组患者治疗前FINS、Homal R、AUC、LH、FSH及T的差别均无统计学意义(P>0.05);治疗后两组患者FINS、LH及T均明显下降(P<0.05),且试验组下降更为明显(P<0.05),试验组Homal R及AUC较治疗前下降(P<0.05),而对照组则无明显差别(P>0.05)。试验组患者的排卵率(81.1%)明显高于对照组(56.8%;χ2=5.110,P=0.024),但两组患者妊娠率差别无统计学意义(35.1%、16.2%;χ2=3.470,P=0.062)。结论吡格列酮联合克罗米芬治疗多囊卵巢综合征具有良好的疗效,可更好地改善机体激素水平,有利于排卵,值得进一步在临床推广。
OBJECTIVE To observe the curative effect of pioglitazone combined with clomiphene in treatment of polycystic ovary syndrome （PCOS） as well as their effect on insulin, sex hormone and ovulation rate. METHODS To select patients undergoing PCOS admitted to our hospital＇s gynecology department from August 2011 to April 2014, and 84 patients were admitted. They were randomly divided into 2 groups. Group 1 was treated with pioglitazone combined with clomiphene, while group 2 was treated with clomiphene only. Both groups were treated for 3 months. To compare the Fasting insulin（FINS）, insulin resistance（Homal R）, area under the curve（AUC）, luteinizing hormone（LH）, follicle-stimulating hormone（FSH）, testosterone（T） levels before and after treatment. RE- SULTS There were no differences in FINS, Homal R, AUC, LH, FSH, T levels between 2 groups before treatment. After the treatment, FINS, LH and T levels all decreased in both groups（P〈0.05）, and group 1 decreased significantly more than group 2 （P〈0.05）. Homal R and AUC decreased in group l（P〈0.05）, but there was no difference in group 2（P〉0.05）. The ovulation rate of group 1（81.1%） was significantly higher than group 2（56.8%; x^2=5.110, P=0.024）, but there was no difference between 2 groups in the pregnancy rate（35.1%, 16.2%;x^2=3.470, P=0.062）. CONCLUSIONS Pioglitazone combined with clomiphene had an excel- lent effect on PCOS, which could improve insulin, sex hormone and ovulation rate. It is worth promoting.
