摘要
目的了解楼宇职业人群健康状况与需求,探索全科医师签约制度下楼宇职业人群健康管理模式。方法自行设计调查问卷,了解上海市花木社区东辰商务楼人驻企业在职员工的基本情况、健康状况、医疗服务利用情况及健康服务需求;由全科医师在集体签约形式下构建健康管理网络,建立健康档案,开展健康教育,组织健康讲座,实施健康检查等健康管理的实践。结果发放问卷550张,回收有效问卷515张,回收有效率93.64%;东辰商务楼内30岁以下职工占52.38%,大专以上学历占89.68%;位列前3项的主要健康问题是抵抗力差、颈脊腰椎疾病、肩周炎等关节疾病;位列前3项的主要不良习惯是缺乏运动、作息不规律、饮食不合理;从来不看病与尽量不看病的人员占45.43%;健康服务需求中位列前3项的是健康知识讲座、就医绿色通道、慢性病筛查;全科医师组织开展的健康管理活动参与率在8%~35%。结论楼宇职业人群有健康管理的主客观需求,但依从性较差,参与率低;楼字职业人群健康管理模式需要创新;建议结合楼宇职业人群年轻化、高学历、流动性、参与率低的特点,采用不占用时间、不固定地点、可自主选择、能互动咨询的移动式管理形式,如利用微信、微博等公众平台,进一步探索全科医师签约制度下楼宇职业人群健康管理模式,使有限的医疗资源发挥最大效应。
[Objective] To understand the health status and demands of building occupational groups, explore the health management mode of occupational groups in building under general practitioner (GP) signing system.[ Methods ] A self-designed questionnaire was used to understand the basic situation, health status, medical service utilization and health service demands of occupational groups in Dongchen Commercial Building of Huamu Community. Based on the collective contract, the general practitioners took the practices of health management, constructed health management network, established health document, carry out the health education, implement the health education lectures, and conduct the health examination. [Results] 515 of 550 questionnaires were completed, with the complete rate of 93.64%. The workers below 30 years old in Dongchen Commercial Building accounted for 52.38%, and employees with college degree or above accounted for 89.68%. The top three major health problems were poor immunity, cervical spinal and lumbar disease, as well as scapulohumeral periarthritis and other joint diseases. The top three bad habits were lack of exercise, irregular rest-work status, and unreasonable diet.Person who never visit doctor and try not to visit doctor accounted for 45.43% .The top three demands for health service were health knowledge lectures, green channel for medical treatment, and chronic disease screening.The participation rates of health management activities implemented by genera/ practitioners were between 8% and 35%. [Conclusion] The occupational groups in building have subjective and objective demands of health management, hut the rates of compliance and participation are low. The health management mode for building occupational groups should be innovated.
