摘要
目的通过检测临床长期服用氯吡格雷的急性冠状动脉综合征(ACS)患者淋巴细胞上多耐药基因1(MDR-1)G2677T/A多态性及表达的P糖蛋白(P-gp),探讨P-gp表达和G2677T/A多态性与ACS之间的相关性。方法运用RT-PCR及测序法检测服用氯吡格雷1年以上的ACS患者(53例)淋巴细胞上G2677T/A表达,采用流式细胞计数(FCM)检测P-gp表达。选取31例健康体检者作为对照组。结果 1病例组G2677T/A阳性率(58.5%)高于健康对照组(3.2%),差异有统计学意义(P<0.05)。2病例组G2677T/A多态性G/G型、T/T型和A/A型的表达率分别为67.7%、22.6%、9.7%,对照组分别为71.4%、14.3%和14.3%;两组各基因型的表达率相比差异均无统计学意义(P>0.05)。3G2677T/A各基因型P-gp表达:T/T型P-gp值为0.240±0.182,A/A型为0.181±0.135,G/G型为0.165±0.142,阴性为0.151±0.126;T/T基因型与阴性相比差异有统计学意义(P<0.05),其余各型与阴性相比差异无统计学意义(P>0.05)。4各基因型的优势比(OR):G/G型为0.317,T/T型为0.408,A/A型为0.029。结论 G2677T/A多态性与长期服用氯吡格雷的ACS或PCI患者多药耐药有一定相关性,G/G基因型P-gp表达量最低,可能为一具有保护性的基因型。
Objective To investigate the relationship between the expression of P-glycolprotein(P-gp), polymorphism of G2677T/A and acute coronary syndrome(ACS) by detecting the polymorphism of MDR-1 G2677T/A and the expression of P-gp in lymphocyte of ACS patients with long-term clopidogrel treatment. Methods The expression of G2677T/A in lymphocyte of ACS patients receiving clopidogrel more than one year was detected by RT-PCR and sequences methods, the expression of P-gp was detected by flow cytometry(FCM). 31 cases of healthy people were set as control group. Results ①The positive rate of G2677T/A in ACS group was higher than that of control group(58.5% vs. 3.2%), with statistical difference(P〈0.05). ②The expression rate of G/G, T/T and A/A genotypes of G2677T/A in ACS group were 67.7%, 22.6% and 9.7% respectively, which had no statistical difference compared with control group(71.4%, 14.3%, 14.3%).③The expression of P-gp of different genotypes of G2677T/A were: T/T: 0.240 ± 0.182, A/A: 0.181 ± 0.135, G/G: 0.165 ± 0.142, negative type: 0.151 ± 0.126; no statistical difference was found between different genotypes and negative type except T/T genotype and negative type. ④The OR of G/G, T/T and A/A genotypes were 0.317, 0.408 and 0.029 respectively. Conclusions The polymorphism of G2677T/A has a certain correlation with the multi-drug resistance of patients with ACS or PCI, and G/G gentype maybe a protective factor because of its lowest P-gp expression.
