Cox-2、Cx43的表达及Hp感染在胃腺癌发生发展中的作用及相关性 被引量:5

The effct of cyctooxygenase-2,connexin43 proteins expression and H·pylori infection to the occurrence of gastric adenocarcinoma and its correlation analysis

摘要 目的:检测慢性浅表性胃炎、慢性萎缩性胃炎、胃腺癌组织中环氧合酶-2(Cox-2)、间隙连接蛋白43(Cx43)的表达及幽门螺杆菌(Hp)感染情况,探讨三者在胃腺癌发生发展中的作用及相互关系。方法:检测不同患者Hp感染情况,采用免疫组织化学染色检测Cox-2及Cx43在胃黏膜标本中的表达水平。结果:Cox-2在慢性浅表性胃炎、慢性萎缩性胃炎、在胃腺癌病理分型及不同TNM分期中的阳性表达率呈明显递增,而Cx43的阳性表达率呈递减趋势,差异有统计学意义。Hp的感染率差异无统计学意义。胃癌组织中Hp感染与Cox-2阳性表达之间存在正相关,与Cx43阳性表达之间存在负相关,Cox-2与Cx43之间的阳性表达存在负相关。结论:Cox-2与胃癌的发生、发展、分化、侵袭浸润过程密切相关。Cx43的表达下调在胃腺癌的发生及发展中起重要作用。Hp感染可通过诱导Cox-2过度表达及降低Cx43阳性表达参与胃腺癌发生发展过程。 Objective: To investigate the expression of cyctooxygenase-2( Cox-2),connexin43( Cx43),and Helicobacter pylori( Hp) in chronic superficial gastritis( CSG),chronic atrophic gastritis( CAG) and gastric adenocarcinoma( GACa) and to analyse the relationships between their expressions and the clinicopathological parameters respectively. Methods: To detect Hp infection in patients,using immunohistochemical staining to detect Cox-2 and Cx43 expression level in specimens of gastric mucosa. Results: Cox-2 in CSG,CAG,GACa pathology classification and positive expression rate of different TNM staging was increasing,and the positive expression rate of Cx43 showed a trend of decline,the difference was statistically significant. There was no statistically significant difference of Hp infection. In stomach tissue Hp infection was positively associated with Cox-2 positive expression,but was nagatively associated with Cx43,and Cox-2 was nagatively associated with Cx43. Conclusion: Cox-2 has certain relations with the gastric cancer occurrence,development,differentiation,invasion. Expression of Cx43 plays an important role in the occurrence and development of gastric adenocarcinoma. Hp infection can be induced by excessive Cox-2 expression and reduce the expression of Cx43 positive to participate the development of gastric adenocarcinoma.
