摘要
目的:研究上消化道出血的严重程度与幽门螺杆菌(Hp)感染及胃癌前疾病的相关性.方法:选取2017年7月-2018年7月本院收治的300例上消化道出血患者,根据改良Forrest分级标准分为Ⅰ、Ⅱ、Ⅲ组,每组100例.所有患者均进行胃镜检查和病理活检,对比上消化道出血的严重程度与幽门螺杆菌感染、胃癌前疾病的相关性,并应用Logistic回归分析上消化道出血的独立危险因素.结果:三组各临床症状发生率、Hp感染的阳性率、胃癌前病变发生率比较,差异均有统计学意义(P<0.05),其中Ⅰ组各临床症状发生率、Hp感染的阳性率、胃癌前病变发生率均显著高于Ⅱ组和Ⅲ组,且Ⅱ组均显著高于Ⅲ组,差异均有统计学意义(P<0.05).多因素Logistic回归分析结果显示,幽门螺杆菌感染、胃癌前病变是上消化道出血发生的独立危险因素(P<0.05).结论:上消化道出血的严重程度与幽门螺杆菌感染及胃癌前疾病存在相关性.
Objective:To study the correlation between the severity of upper gastrointestinal hemorrhage and Helicobacter pylori(Hp)infection and precancerous diseases.Method:300 patients with upper gastrointestinal bleeding admitted to our hospital from July 2017 to July 2018 were selected,according to the grading standards improved Forrest into Ⅰ,Ⅱ,Ⅲ group,100 cases in each group.All patients underwent gastroscopy and pathological biopsy,the correlation between the severity of upper gastrointestinal bleeding and Hp infection and pregastric cancer was compared,independent risk factors of upper gastrointestinal bleeding were analyzed by Logistic regression.Result:Comparison of clinical symptom incidence,positive rate of Hp infection and incidence of pregastric cancer among the three groups showed statistically significant differences(P<0.05). Including Ⅰ group,the incidence of clinical symptoms,the positive rate of Hp infection,the incidence of pathological changes before gastric carcinoma were significantly higher than those of Ⅱ group and group,and Ⅱ group were significantly higher than those of Ⅲ group,the differences were statistical significance(P<0.05). Multivariate Logistic regression analysis showed that Hp infection and gastric prelesions were independent risk factors for upper gastrointestinal bleeding(P<0.05).Conclusion:The severity of upper gastrointestinal bleeding is correlated with helicobacter pylori infection and pregastric cancer.
