摘要
目的分析交锁髓内钉配合自主体位治疗模式在股骨骨折中的临床疗效。方法选取2011年9月至2012年8月遵化市人民医院骨科收治的股骨骨折患者98例,采用投掷硬币法随机分为观察组和对照组,各49例。观察组采用交锁髓内钉内固定治疗后给予自主体位配合协助体位治疗,对照组采用髓内钉内固定治疗后给予协助体位治疗。比较两组患者的治疗效果、髋关节和膝关节功能、负面情绪评分等。结果观察组总有效率为98.0%,对照组为87.8%,观察组临床疗效显著优于对照组(P<0.05)。观察组Harris评分、HSS评分分别为(65±7)分、(90±9)分,显著高于对照组的(51±5)分、(76±8)分(P<0.05)。观察组视觉模拟评分、汉密尔顿抑郁量表评分、汉密顿焦虑量表评分显著低于对照组[(2.4±0.4)分比(4.6±1.6)分,(18.6±2.6)分比(37.6±5.4)分,(14.4±1.5)分比(39.3±4.2)分],差异均有统计学意义(P<0.01)。结论交锁髓内钉配合自主体位治疗模式可以有效提高股骨骨折患者的治疗效果,改善关节功能,降低负面情绪。
Objective To analyze the efficacy of interlocking intramedullary nail with independent pos-tural mode in femoral fractures.Methods A total of 98 patients with femoral fractures admitted to Depart-ment of Orthopedics,Zunhua People′s Hospital were divided into observation group and control group accord-ing to the coin tossing method,49 cases in each group.Patients in the observation group were given intramed-ullary screw fixation and independent postural therapy , while patients in the control group were given intr-amedullary screw fixation and assisted postural therapy.Differences of therapeutic effect,the hip and the knee function and negative mood scores of the two groups were compared.Results The overall effective rate in the observation group was 98.0%,and in the control group was 87.8%,the difference was statistically signifi-cant(P〈0.05).Harris score and HSS scores of the observation group were significantly better than the con-trol group[(65 ±7)scores vs(51 ±5)scores,(90 ±9)scores vs(76 ±8)scores,P 〈0.05].VAS,HAMD, HAMA of observation group were significantly lower than the control group[(2.4 ±0.4)scores vs(4.6 ±1.6) scores,(18.6 ±2.6)scores vs(37.6 ±5.4)scores,(14.4 ±1.5)scores vs (39.3 ±4.2) scores,P〈0.01]. Conclusion Interlocking intramedullary nail with independent postural mode can effectively improve the therapeutic effect and joint function ,and reduce the negative mood of patients with femoral fractures .
