摘要
目的系统评价机械刺激子宫内膜后对改善体外受精-胚胎移植结局的效果。方法计算机检索Cochrane library、EMBASE、Pub Med、Springer、CNKI、VIP及万方等数据库,搜索2004年至2014年发表的有关机械刺激子宫内膜与体外受精-胚胎移植的随机对照试验与非随机对照研究,同时检索纳入文献的参考文献。按Crochrane系统评价方法进行纳入文献的选择和质量评价并采用软件Rev Man5.0进行分析。结果共纳入9篇对照研究,涉及受试者1398例。Meta分析结果显示:子宫内膜机械刺激组胚胎种植率[RR=1.60,95%CI(1.25,2.05),P=0.0002]、临床妊娠率[RR=1.77,95%CI(1.48,2.12),P<0.00001]及活产率[RR=1.93,95%CI(1.45,2.58),P<0.00001]均高于对照组且具有统计学意义。结论对行体外受精-胚胎移植助孕者于移植前机械刺激子宫内膜,有助于提高胚胎种植率和临床妊娠率。
0bjective：To analyze the effect of endometrial superficial mechanical stimulation on the incidence of successful pregnancy in patients undergoing in vitro fertilization-embryo transfer（IVF—ET）.Methods：Databases such as Pub Med,Springer,ISI,EMBASE,Cochrane Library,CNKI,VIP were searched for the identification of randomized controlled trials（RCT）and non-randomized comtrolled studies evaluating the endometrial superficial mechanical stimulation before or during ovarian stimulation in vitro fertilization-embryo transfer（IVF-ET）from 2004 to 2014. The methodological quality of the included RCT was assessed,and the data were extracted according to the Cochrane Handbook.Meta-analysis was performed by using Rev Man 5.0 software.Results：There were no significant differences between the two groups in the age of the patients,response to hormone stimulation,the numbers of oocytes and embryo,and the rate of fertilization.However,the rates of clinical pregnancy[RR=1.77,95%CI（1.48,2.12）,P〈0.00001],embryo implantation[RR=1.60,95%CI（1.25,2.05）,P=0.0002] and Live births[RR=1.93,95%CI（1.45,2.58）,P〈0.00001] in the treatment group were significantly higher than those in the control group.Conclusion：Endometrial superficial mechanical stimulation significantly improves the successful pregnancy in patients undergoing in vitro fertilization.
