摘要
目的：利用免费婚检服务平台，开展艾滋病咨询检测，对检出艾滋病（HIV）阳性者进行婚育指导，从而减少艾滋病平行传播和母婴传播。方法：对免费婚检对象同时进行婚前医学检查和艾滋病咨询检测，查出艾滋病（HIV）阳性者进行婚育指导，单方阳性者告知艾滋病的危害及传播方式，预防措施等知情选择，坚持结婚的建议同房戴安全套，双方均为阳性者建议采取避孕措施，坚持生育者将其纳入艾滋病系统管理，对HIV阳性母亲、婴儿在孕期、围生期综合应用抗逆转录病毒药物、选择性剖宫产和人工喂养等措施，避免艾滋病平行传播和母婴传播。结果：2012～2013年全区免费婚检对象19932对39864人，全部参加艾滋病咨询检测，平均婚检率95．28％，艾滋病咨询检测率同样是95．28％，检出艾滋病（HIV）阳性者92人，其中单方阳性74人（男方阳性52人，女方阳性22人），男女双方阳性10对20人，通过婚育指导后3—6个月对其配偶进行追踪回访检测，所有男女单方艾滋病（HIV）阳性者的配偶均未发现艾滋病（HIV）阳性者；艾滋病（HIV）阳性妇女怀孕者16人，经婚育指导后终止妊娠2人，其余14人均纳入艾滋病系统管理体系，按艾滋病母婴传播技术方案应用抗逆转录病毒药物预防。在婴儿出生时及出生后1、3、6、9、12和18个月对其进行艾滋病（HIV）抗体检测，均未检出艾滋病（HIV）阳性者。结论：利用免费婚检服务平台，开展艾滋病咨询检测，对检出艾滋病（HIV）阳性者进行婚育指导并将其纳入艾滋病系统管理，对预防艾滋病平行传播和母婴传播有重要意义。
Objective： Using of free premarital examination service platform, to carry out AIDS counseling detection, to carry out marriage and childbirth guidance for the HIV positive persons, in order to reduce AIDS parallel transmission and MTCT. Methods： Free premarital examination objects accepted premarital medical examination and AIDS counseling and testing. Marriage and childbirth guidance was carried out for HIV positive persons. Dangers of AIDS, mode of transmission of AIDS, preventive measures of AIDS and so on were taught to unilateral positive persons. For adbereing to the marriage, persons should be suggested to wearing a condom intercourse. Persons of two sides were positive should be suggested using contraception, who insisting on fertility should be included in the AIDS system manage- merit. For HIV positive mothers and infants in the during pregnancy and perlnatal, comprehensive use of anti retroviral drugs, selective cesar- ean section and artificial feeding and other measures were done to avoid AIDS parallel transmission and MTCT. Results： From 2012 to 2013, there were 19 932 pairs, 39 864 persons who were the free premarital examination objects, all of them took part in the HIV counseling and testing, the average rate of premarital examination was 95. 28%, HIV counseling and testing rate was the same 95.28%, there were 92 AIDS （HIV） positive persons, including 74 unilateral positive persons （ male was 52, female was 22）, men and women both parties posi- tive were 10 pairs, 20 persons. Through the marriage and childbirth guidance after 3 ~ 6 months, tracking return visit was used to test their spouses. All unilateral AIDS （HIV） positive patient had not been found their spouse positive for HIV/AIDS, there were HIV positive preg- nant women （HIV） 16 persons, termination of pregnancy 2 persons after marriage and childbirth guidance, the rest 14 persons were all in- cluded in the AIDS system management system and treated with antiretroviral drugs according to the scheme of HIV mother-to-
