基底神经节区脑血管病变致记忆障碍的特点分析 被引量:3

Clinical feature analysis of memory disorder due to cerebrovascular lesions in basal ganglia

摘要 目的观察左、右两侧基底神经节区脑血管病变所致记忆障碍的特点。方法选取左侧基底神经节区脑血管病变患者(A组)和右侧基底神经节区脑血管病变患者(B组)各50例为观察对象,应用修订的韦氏成人记忆量表对患者进行全面记忆功能检查,记录各项粗分及量表分,计算记忆商数并进行统计分析,确定记忆功能障碍类型。结果 A组患者的记忆障碍以语文记忆障碍为主、B组患者的记忆障碍以非语文记忆障碍为主,A组的记忆障碍以短时记忆障碍为主、B组的记忆障碍以图形、视觉空间记忆障碍为主。结论左侧基底神经节区脑血管病变患者的记忆功能改变明显,主要影响与语文相关的记忆功能,提示左侧基底神经节区在记忆方面存在偏侧化优势;右侧基底神经节区脑血管病变患者的图形、视觉空间记忆损害更明显,提示右侧基底神经节区与右侧大脑半球一样在视空间功能方面占优势。 Objective To observe the difference of the memory disorder between the right and left basal ganglia lesions. Methods 50 cases in each group were observed,with single lesion in the left basal ganglia area (A group) and in the right basal ganglia area (B group). The memory functions of patients were tested with revised Wechsler Adult memory scale, the scores were recorded, memory quotients were calculated and statistical analyzed to determine the type of memory dysfunction. Results The disorders of memory in A group were mainly verbal memory and short-term memory disorder, in B group were mainly in non-language memory and graphic, visual spatial memory disorders. Conclusion The memory function changed significantly in patients with left basal ganglia area lesion, mainly affecting the memory function associated with the language,suggesting that the left basal ganglia area has the laterality advantage in memory. In the group with right basal ganglia lesion, graphic, visual-spatial memory deficits damaged more obviously, indicating that the right basal ganglia area, like the right hemisphere, has dominant in visuospatial function.
