摘要
目的研究血管性认知功能障碍(VCI)的中医证型、神经心理学及影像学特点,探讨其意义。方法将107例脑血管病患者分为对照组、血管性痴呆组(VD组)及血管性认知功能障碍非痴呆组(VCIND组),对三组患者分别进行证候学类型研究,研究神经心理学测试和颅脑CT检查。结果中医证候积分的Logistic回归分析显示,肾精亏虚、热毒内盛、肝阳上亢三个证候与VCI显著相关。VCI认知功能各分测验成绩的标准化Z值显示,VD组和VCIND组相对于对照组变化幅度最大的是连线测验,变化幅度最小的是四个词的记忆测验。颅脑影像学检查指标的多因素分析显示,白质疏松积分和双额纵裂宽度与VCI显著相关。结论肾精亏虚证和热毒内盛证是认知功能下降的危险因素。VCI以额叶功能和注意力损害较突出,记忆损害较轻,具有不均质性的特点。脑白质疏松与VCI的发病关系更为密切。
Objective To investigate the syndromes of Chinese medicine,neuropsychology and imaging characteristic in vascular cognitive impairment（VCI）and explore their significance.Methods 107 cases of cerebrovascular disease were divided into control group,vascular dementia group（VD group）and non-dementia of VCI group（VCIND group）.The study on syndrome types,test of neuropsychology and CT examination of cranium were conducted.Results The logistic stepwise regression analysis of syndrome integral of Chinese medicine showed that three syndromes were relevant significantly with VCI, named deficiency of kid- ney essence,internal excess of heat toxin and hyperactivity of liver yang. Sub - test scores of standardized Z value on cognition in VCI indicated that the greatest changes in line connection test, and the smallest changes in four - word memory test happened in VD group and VCIND group compared with control group. The multivariate analysis in cranial imaging examination explained that leukoaraiosis integral and width of bilateral frontal longitudinal fissure were relevant significantly with VCI. Conclusion Deficiency of kidney essence and internal excess of heat toxin are the risk factors of decreased cognition. VCI is characterized by severe damage of prefrontal lobe function and attention, mild memory damage and heterogeneity. The leukoaraiosis is especially closely related to VCI.
