摘要
目的探索模拟家庭医生契约服务在全科医生规范化培训中的可行性及应用价值。方法选择2013年2月~2014年2月同济大学附属杨浦医院全科基地规范化培训学员共计16名,随机分为两组。实验组8名学员,按照大纲轮岗培训之外,参加模拟家庭医生契约服务项目。对照组8名学员,按照大纲轮岗培训。每2名实验组学员配备1名固定的带教老师。在老年科住院患者中,选择病情稳定的患者32例,分别对应32个家庭,以1个实验组学员分配4个家庭为标准,签订模拟契约。学员与契约对象每个月主动电话联络1次,每3个月家访1次,对每名家庭成员进行健康咨询以及慢性病随访,同时发放相应健康宣教材料。跟踪随访1年后,课题组通过问卷调查,向契约服务对象了解其对家庭医生的评价。另一方面,通过临床轮岗科室带教老师的综合评估,对两组学员实验前后临床业务能力及人文关怀、沟通能力进行评价。对实验组学员进行反馈调查。结果契约服务对象对家庭医生服务的总体评价是比较满意的。统计显示,两组实验后临床业务能力良好率差异无统计学意义(P〉0.05);两组实验后人文关怀、沟通能力良好率差异有统计学意义(P〈0.05)。轮岗各科室带教老师综合评价显示,实验组学员人文沟通能力强于对照组学员。模拟契约服务对于增强人际沟通能力、强化理论联系实践很有帮助。结论模拟契约服务对提升全科学员人文关怀和沟通能力有意义,模拟契约服务可行。
Objective To explore the feasibility and value of analog-contracts-services in standardized general practitioners training project. Methods In Yangpu Hospital of Tongji University, a total of 16 general standardization trainees were randomly divided into 2 groups from February 2013 to February 2014. 8 participants in the control group had been scheduled to rotate traditionally. One fixed tutor guided 2 students in the experimental group. 8 participants in the experimental group had been scheduled to rotate to participate in general practitioners analog-contracts-services. 32 stable patients in hospitalized geriatric ward which from 32 families respectively were signed analog-contracts-services. Each trainee assigned 4 families as standard in the experimental group. General practitioners trainees called the family per month, visited the home 1 time every 3 months to do health advice and follow-up of chronic diseases, and issuance of appropriate health education materials. Follow-up 1 year later, the study group collected the final evaluation towards their general practitioners trainees from every family through questionnaires. On the other way, the guided tutors also e- valuated the general practitioners trainees during clinical rotation to obtain their comprehensive, clinical operational capacity, human values and communication skills before and after the experiment on two groups of trainees. The results of evaluation were returned to the general practitioners trainees in the experimental group. Results The overall evalua- tion from the family were fairly satisfied with the analog-contracts-services. Statistics showed that experimental clinical operational capacity had no significant difference between the two groups (P 〉 0.05), but humane care and good com-munication skills had significant differences (P 〈 0.05).
