摘要
目的探讨综合保温措施对老年患者前列腺电切围手术期体温的影响。方法选取100例年龄在65岁以上的前列腺电切手术患者,按住院号单双随机分为观察组(单号)和对照组(双号),每组各50例。对照组术中的冲洗液加温至37-39℃,其余常规处理;观察组在对照组保温措施基础上,采用术前1 h静脉给予支链氨基酸3.5 ml/kg,患者头部避开天花板垂直送风下方,采用充气式保温毯,术中使用液体升温仪进行输液(液体温度为37℃)等综合保温措施。观察两组患者围术期腋下温度及寒战和躁动的发生率。结果对照组及观察组手术开始后30 min体温均下降,但观察组体温(35.5±0.29)℃明显高于对照组(35.9±0.30)℃(P<0.05)。手术结束时,低体温发生率观察组(48%)明显高低于对照组(18%)。观察组围术期寒战、躁动的发生率较对照组明显减少(P<0.05)。结论围术期积极采用综合保温措施可减少老年前列腺电切手术患者低体温的发生,降低围术期寒战、躁动的发生。
Objective To investigate the influence of comprehensive thermal insulation measures on perioperative hypothermia in senile patients undergoing transurethral resection. Methods According to the even-odd admission number,100 patients over 65 years old undergoing transurethral resection were randomly divided into observation group and control group with 50 patients in each group.The washing liquid was heated to 37- 39 ℃ in control group. Based on the use of heat preservation measures,the patients in observation group were given intravenously chain amino acid(3. 5 ml / kg) at 1 h before the operation,the heads of the patients were far away from the air outlet,and the comprehensive thermal insulation measures were made,including inflatable thermal insulation blanket,liquid infusion heating apparatus in operation( liquid temperature of 37 ℃). The perioperative axillary temperature and the incidence of shiver and the irritability were observed. Results The temperature of the patients in control group and observation group decreased at30 min after the beginning of the operation,but it was significantly higher in observation group than in control group( P〈0. 05). The incidence of hypothermia was lower in observation group than that in control group at the end of operation. The incidence of shiver and irritability was lower in observation group than that in control group( P〈0. 05). Conclusion Perioperative comprehensive thermal insulation measures can reduce the incidence of low temperature,and prevent from the shiver and irritability in senile patients with transurethral resection.
