摘要
目的探讨酪酸梭菌活菌散联合乳果糖对儿童功能性便秘(FC)患者血清胃肠激素水平的影响及疗效。方法选取儿科门诊就诊的儿童FC患者78例,随机分为研究组和对照组,每组39例。对照组患儿均予以多饮水、调整饮食和加强排便锻炼等常规治疗。研究组患儿在此基础上加用口服酪酸梭菌活菌散联合乳果糖口服液治疗4周,其中酪酸梭菌活菌散0.5 g/次,2次/d;乳果糖口服液5 m L//次,2次/d。观察并记录两组患儿治疗前和治疗4周后血清胃肠激素P物质(SP)和生长抑素(SS)水平的变化,并评估其临床效果及不良反应。结果治疗4周后,两组患者血清SP或SS水平较前明显上升或较前明显下降,且研究组变化幅度较对照组更明显(P<0.05);同时在临床总有效率方面研究组患儿(94.87%)较对照组更佳(79.49%)(χ2=4.13,P<0.05),研究组治疗中出现不良反应4例,症状均较轻。结论酪酸梭菌活菌散联合乳果糖治疗儿童FC的疗效较显著,安全性较好,其作用机制与其能调节胃肠激素的分泌异常,升高血清SP水平和降低血清SS水平,加快胃肠蠕动功能密切相关。
Objective To discuss influence and curative effect observation of Clostridium butyricum Powder combined with Lactulose on serum gastrointestinal hormone of children with functional constipation（FC）. Methods 78 cases of children with FC, who were given the medical treatment at outpatient in Department of Paediatrics, were selected, and divided into research group and control group at random, with 39 cases of children in either group. The children in control group were given routine medical treatment, such like plenty of water, diet adjust- ment, enhancement of stool exercise and etc, and the children in research group were additionally given oral Clostridium butyricum Powder combined with Lactulose for 4 weeks, which included 0. 5 g Clostridium butyricum Powder per time, twice a day and 5 mL Lactulose oral liquid per time, twice a day. The changes of serum gastrointestinal hormone Substance P（SP） and somatostatin（SS） levels of children in two groups before and 4 weeks after medical treatment were observed and recorded, and the clinical curative effect and untoward effect were evaluated as well. Results After 4 weeks＇ medical treatment, serum SP level of children in two groups obviously rose than before, while SS level obviously declined than before, and the change rate of children in research group was much higher than those in control group （ P 〈 0.05 ）. Meanwhile, total clinical efficiency of children in research group （94. 87% ）was much higher than that in control group （79. 49% ） （χ^2 = 4. 13, P 〈 0.05 ）. 4 cases of untoward effect appeared in research group during the medical treatment with light symptom. Conclusion Clostridium butyricum Powder combined with has significant curative effect on children with FC with high security, whose mechanism of action has close effect on adjusting the abnormal secretion of gastrointestinal hormone, raising serum SP level and lowering serum SS level and speeding the gastrointestinal motility function.
