摘要
目的分析全口义齿疼痛原因,寻求处理方法。方法选取2013年3月~2014年8月期间全口义齿疼痛的患者共231例,结合检查结果和病例资料分析全口义齿疼痛原因,根据患者处理方法的不同分为对照组110例与治疗组121例,对照组调磨疼痛区,治疗组调磨疼痛区、支点和早接触点,缓戴牙1 d,重新排牙;两组均使用浓替硝唑含漱液口腔含漱,对比两组患者的临床疗效。结果定位明确的义齿疼痛原因包括基托边缘过长或过锐、基托进入组织倒凹、缓冲区缓冲不足、局限性咬合高点,4种疼痛因素构成比依次递减;定位不明确疼痛原因包括垂直距离过高、正中关系错误、咀嚼压力过大、基托不密合,4种疼痛因素构成比依次递减。治疗组积极治疗后疼痛显著缓解率72.7%、总缓解率98.3%,均明显高于对照组的47.3%和91.8%（P〈0.05）。结论全口义齿疼痛表现不同、原因也不尽相同,根据疼痛表现结合病历资料和检查结果明确疼痛原因后,缓戴牙、调磨疼痛区、支点和早接触点,配合浓替硝唑含漱,有显著控制效果。
Objective to analyze the causes of complete denture pain, and seek treatment methods. Methods 231 patients with complete denture pain were selected to analyze pain causes from March 2013 to August 2014. The patients were divided into control group（n = 110）and treatment group（n =121）according to the different treatment. The patients in the control group were with grinding pain, while the patients in the treatment group were with grinding pain, fulcrum and premature contacts, corrosion wear teeth one day and the new row of teeth. Results The denture pain causes of clear positioning include b ase edge too long or too sharp, base into the tissue undercut, buffer, buffer insufficient limitations of occlusal disturbance; the denture pain causes of unclear positioning include high vertical distance, median relative error, over masticatory pressure, base not closed. The treatment group was better than control group in pain relief. Conclusion The causes of complete denture pain are different because of the clinical features, then the pain would be relieved, when we have the complete denture done with method like corrosion wear, grinding tooth pain area, fulcrum and premature contacts, and tinidazole gargle.
