摘要
目的探讨习惯性流产(RSA)患者绒毛和蜕膜中Toll样受体4(TLR4)与Treg/TH17细胞免疫平衡的关系。方法收集32例RSA患者和32例健康对照组的绒毛和蜕膜,用实时荧光定量PCR法检测其TLR4 mRNA表达水平,用流式细胞术检测Treg和TH17细胞数量,分析TLR4 mRNA与Treg/TH17细胞间的相关性。结果 RSA患者绒毛和蜕膜中TLR4 mRNA的相对表达水平分别为2.6±1.1、2.1±1.1,明显低于健康对照组(7.4±2.3、6.8±1.3),差异有统计学意义(t分别为10.650、15.613,P均<0.01)。Treg细胞百分率分别为(7.75±1.83)%、(7.24±1.71)%,明显低于健康对照组(12.18±2.65)%、(11.64±3.23)%,差异有统计学意义(t分别为7.781、6.810,P均<0.01);TH17细胞百分率分别为(7.78±2.13)%、(8.13±2.11)%,明显高于健康对照组(3.85±1.53)%、(4.11±1.52)%,差异有统计学意义(t分别为8.477、8.13,P均<0.01)。RSA患者绒毛和蜕膜中TLR4 mRNA水平均与Treg细胞数量呈正相关(r分别为0.925、0.948,P均<0.01);RSA患者绒毛和蜕膜中TLR4 mRNA水平均与TH17细胞数量呈负相关(r分别为-0.954、-0.939,P均<0.01)。结论 RSA患者绒毛和蜕膜中TLR4低表达与Treg/TH17免疫失衡有关,可能参与RSA的发生。
Objective To investigate the correlation between Toll-like receptor 4( TLR4) and Treg / TH17 cells in villus and deciduas of the patients with recurrent spontaneous abortion( RSA). Methods The villus and deciduas were collected from 32 patients with RSA and 32 normal pregnant women. The mRNA levels of TLR4 were determined with real-time fluorescent quantitative PCR. The percentages of Treg and TH17 cells in the villus and deciduas were measured with flow cytometry. The correlations between TLR4 mRNA and Treg / TH17 cells were analyzed. Results The relative expression level of TLR4 mRNA in villus and deciduas of RSA patients were( 2. 6 ± 1. 1) and( 2. 1 ± 1. 1) respectively,which were significantly lower than those of normal pregnant women [( 7. 4 ± 2. 3) and( 6. 8 ± 1. 3) ]( t = 10. 650 and 15. 613,P〈 0.01). The percentages of Treg cells in villus and deciduas were( 7. 75 ± 1. 83) % and( 7. 24 ± 1. 71) % respectively,which were significantly lower than those of normal pregnant women [( 12. 18 ± 2. 65) % and( 11. 64± 3. 23) % ]( t = 7. 781 and 6. 810,P〈 0.01). The percentages of TH17 cells were( 7. 78 ± 2. 13) % and( 8. 13 ± 2. 11) % respectively,which were obviously lower than those of normal pregnant women [( 3. 85 ± 1. 53) % and( 4. 11 ± 1. 52) % ]( t = 8. 477 and8. 745,P〈 0.01). There was strongly positive correlation between the levels of TLR4 mRNA and the percentages of Treg cells in villus and deciduas from RSA patients( r = 0. 925 and 0. 948,P〈 0.01),while a remarkably negative correlation between the levels of TLR4 mRNA and the percentages of TH17 cells was found in villus and deciduas of RSA patients( r =- 0. 954 and-0. 939,P〈 0.01). Conclusion The low expression of TLR4 mRNA was related to the imbalance of Treg / TH17 cells in villus and deciduas of RSA patients,which may participate in the immunopathogenesis of RSA.
