川芎嗪对脑梗死患者血流变及血小板指标的影响 被引量:5

摘要 目的:观察川芎嗪治疗急性脑梗死的疗效,及其对患者血流变及血小板相关指标的影响。方法:将240例急性脑梗死患者随机分为两组,对照组和观察组,各120例,对照组采用西医基础治疗,观察组患者在此基础上加载盐酸川芎嗪注射液。采用NIHSS量表和Rankin量表对两组患者的神经功能缺损程度和残障水平进行评定,比较两组间疗效差异。治疗前后对两组患者的血流变学及血小板相关指标进行检测,比较组间差异。结果:治疗14d后,观察组近期疗效显著优于对照组(P<0.05);疗后第7、14天,观察组患者血细胞压积、血浆黏度、全血高切黏度、全血低切黏度及红细胞聚集指数均显著低于对照组(P<0.05);治疗后第7、14天,观察组患者血小板分布宽度(PDW)、血小板平均体积(MPV)、血清D-二聚体(D-D)、纤维蛋白酶原(FIB)均显著低于对照组(P<0.05)。结论:川芎嗪能有效改善脑梗死患者血流变和血小板相关指标,从而改善急性脑梗死预后,降低病残率。 Objective:: To observe the efficacy of tetramethylpyrazine (TMP) for patients with acute cerebral infarction (ACI), and its influence on the hemorheological and platelet indices. Methods:A total of 240 with ACI were randomly divided into control group and observation group, 120 cases in each group. Patients in control group were treated by basic treatment, and those in observation group were treated with [igustrazine hydrochloride injection. Then the neurological impairment and disability level of two groups were detected by NIHSS scale and Rankin Scale. And the indexes of hemorheological and platelet of two groups were detected and compared before and after treatment. Results:After 14 days of treatment, the term effect of observation group was better than control group, P〈0.05. After 7, 14 days of treatment, the indexes of hemorheologieal such as hematocrit, plasma viscosity, high shear whole blood viscosity, low shear whole blood viscosity and red blood cell aggrega- tion index of observation group were all lower than control group, P〈0.05; and the indexes of platelet such as PDW, MPV, D-D, FIB lower than control group too, all P〈0.05. Conclusions::TMP can effectively improve the indexes of hemorheological and platelet, so to improve the prognosis of ACI and reduce the morbidity.
