摘要
目的探讨气管插管患者口腔护理的有效方法,提高护理质量,减少并发症的发生。方法采用对照研究的方式,将2013年1月至2014年6月间我院ICU收治的60例气管插管患者随机分为观察组和对照组,观察组施行负压式牙刷刷牙法,对照组施行常规口腔护理法。比较两组患者护理前后口腔细菌检测情况、口腔症状、pH值、呼吸机相关性肺炎、意外脱管、机械通气的时间以及患者的舒适度。结果观察组护理后即刻的细菌数、护理后2h、4h、6h的细菌数均少于对照组,5d牙菌斑率、口腔细菌阳性率均低于对照组,差异有统计学意义( P<0.05)。观察组护理后的口腔溃疡、疱疹、口臭、呼吸机相关性肺炎的发生率低于对照组,护理5 d时的口腔pH值高于对照组,机械通气时间短于对照组,差异有统计学意义( P<0.05)。两组的舒适度差异无统计学意义( P>0.05)。结论带负压式牙刷刷牙法能够有效清除气管插管患者的口腔细菌,预防感染的发生,优于传统的口腔护理方法。
Objective To explore the application effect of toothbrush brush with negative pressure in oral care of trachea cannula patients, to increase nursing quality, decrease the rate of complications.Methods 60 patients with tracheal intubation were randomly divid-ed into the observation and the control groups.Patients in the observation group were underwent toothbrush brush with negative pressure, in the control group were received conventional oral nursing method.Oral bacteria detection, oral symptoms, pH, ventilator-associated pneu-monia, accidental decannulation, mechanical ventilation time and comfort were compared between two groups.Results Bacteria count of im-mediately, 2 h, 4 h, 6 h after nursing in the observation group after nursing were lower than those of control group, dental plaque rate, posi-tive rate of oral bacteria after 5 d were lower than those of control group, there were all significant differences ( P〈0.05) .Rate of oral cavi-ty ulcer, herpes, halitosis, ventilator-associated pneumonia after nursing in the observation group were lower than those of control group, o-ral pH after nursing 5 d was higher than that of control group, the mechanical ventilation time was shorter than that of control group, there were all significant differences (P〈0.05) .There was no significant difference in comfort between two groups (P〉0.05) .Conclusions Toothbrush brush of negative pressure method could effectively remove the tracheal intubation in patients with oral bacteria, prevent infection;the oral care effect is better than that of traditional care.
