摘要
目的探讨不同剂量来曲唑促排卵中对性激素水平的影响。方法回顾性分析230名不孕症患者的临床资料。其中,采用来曲唑促排卵者172人,按剂量分别为2.5mg/d(n=60)、5mg/d(n=62)、7.5mg/d(n=50)三组,在月经第五天服药,服用5d;采用自然周期监测排卵者58人(对照组)。所有患者均于月经周期第3天(D3)测定基础卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、睾酮(T)水平;来曲唑组用药5d后及对照组均于月经周期第10天(D10)再次测定激素水平;所有患者均定期监测卵泡大小以及排卵情况。结果 (1)与基础水平(D3)比较,来曲唑2.5mg/d、5mg/d组用药后(D10)LH、T水平显著升高(P<0.05),而FSH、E2水平无显著变化(P>0.05);7.5mg/d组用药后FSH、LH、T水平均显著升高(P<0.05),而E2水平无显著变化(P>0.05);(2)来曲唑各剂量组与对照组比较,三组用药后(D10)的E2水平均显著降低,LH、T水平均显著升高,差异有统计学意义(P<0.05);2.5mg/d、5mg/d组FSH水平变化不明显,无统计学差异(P>0.05),7.5mg/d组FSH明显升高,差异有统计学意义(P<0.05)。结论来曲唑促排卵影响到患者的激素水平,其中对FSH水平的影响呈剂量依赖性。
Objective: To evaluate the impact of letrozole on the sex hormone levels in ovulation induction in infertile patients. Methods: A total of 230 infertile patients were divided into four groups, three experimental groups received different doses of letrozole E2. 5 mg/day(n=60),5 mg/day(n=62),7. 5 mg/day (n=50)-] on the fifth day of menstrual cycle for 5 days and one control group without treatment(n= 58). The levels of FSH,LH,E2 and testosterone were measured on the 3rd day and the tenth day of the menstrual cycle. The growth of the follicles until ovulation was monitored after the tenth day of the menstrual cycle. Results: FSH and E~ levels were not significantly different (P 〉 0.05), but LH and T levels significantly increased after treatment compared with those on the 3raday in the group treated with letrozole 2.5 mg/day or 5 mg/day(P〈0.05). Compared with control group, the sex hormones on the 10th day were metabolic,the levels of FSH were significantly increased in the group treated with 7.5 mg/d of letrozole (P〈0.05), the levels of LH and testosterone were significantly increased (P〈0.05), but E2 were significantly decreased in three groups (P〈0.05). Conclusions: In the process of inducting ovulation, the letrozole affects FSH levels in a dose- dependent manner and LH & testosterone levels in a dose-independent manner, and no significant effect on E2 levels.
