摘要
目的了解青岛地区结核分支杆菌KatG315突变特征及其与异烟肼耐药关系。方法应用PCRDNA测序法,对来自青岛地区58株异烟肼耐药的结核分支杆菌和10株异烟肼敏感的结核分支杆菌的KatG基因片段(包含KatG315)进行分析。结果 10株异烟肼敏感菌株均未检测到KatG315突变,而58株异烟肼耐药菌株中检测到26株发生了KatG315突变,突变率为44.83%;异烟肼耐药菌株KatG315突变率高于敏感菌株,差异有显著性(χ2=5.484,P<0.05);异烟肼高浓度耐药菌株的KatG315突变率高于低浓度耐药菌株,差异有显著性(χ2=7.821,P<0.05);多耐药菌株的KatG315突变率高于单耐异烟肼菌株,差异有显著性(χ2=3.604,P<0.05)。结论KatG315突变是青岛地区结核分支杆菌对异烟肼耐药的主要机制。
Objective To investigate the relationship between isoniazid-resistance and mutation in KatG315 of mycohac- terium tuberculosis (MT) in Qingdao area. Methods Using PCR-DNA sequencing, the KatG gene fragmen-including KatG315- of 58 strains of Mycohacterium tuberculosis resistant to isoniazid and 10 strain sensitive to isoniazid collected in Qingdao area were analyzed. Results No mutation of KatG315 was detected in 10 isoniazid-sensitive strains, but in 58 isoniazid-resistant strains, mutation of KatG315 was noted in 26 strains, the mutation rate being 44.83~. The mutation rate of KatG315 in isoniazid-resistant strain was significantly higher than that of isoniazid-sensitive strain (X 2 = 5. 484, P ~ 0.05) ~ that in high-concentration-resistant strain was higher than that in low-concentration-resistant strain (X2= 7.821 ,P〈0.05), and that in multidrug-resistant strain was higher than that in single-isoniazid-resistant strain (X2= 3. 604, P 〈0.05). Conclusion The mutation of KatG315 is the main mechanism of Mycobacterium tuberculosis resistant to isoniazid in Qingdao area.
