无抽搐电休克治疗对老年抑郁症病人的效果 被引量:10

摘要 目的探讨无抽搐电休克治疗(MECT)对老年抑郁症病人的疗效和安全性。方法对45例老年抑郁症病人行MECT,治疗前后采用汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评估病人抑郁严重程度,用韦氏记忆量表(WMS)和简易精神状态检查表(MMSE)检测病人记忆功能和认知功能。结果 MECT治疗结束时,老年抑郁症病人HAMD和HAMA评分显著下降(t=3.94~22.09,P〈0.01),WMS中再认、联想评分及MMSE评分显著下降(t=2.22~2.69,P〈0.05)。治疗结束1月后,老年抑郁症病人MMSE评分较治疗前显著上升(t=2.26,P〈0.05),WMS评分与治疗前比较差异无显著性(P〉0.05)。结论 MECT导致的认知功能损害是暂时的,该治疗对老年抑郁症病人安全有效。 Objective To assess the efficacy and safety of modified electroconvulsive therapy (MECT) for depressed el- derly patients. Methods Forty-five elderly patients with depression were treated with MECT. Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA) were adopted to assess the curative effect by comparing the severity of depression before and after treatment. The effect of MECT on memory and cognitive function was evaluated with Wechsler Memory Scale (WMS) and Mini-mental State Examination (MMSE). Results HAMD and HAMA scores decreased significantly after MECT (t = 3.94-22.09 ,P〈0.01), recognition and association of WMS and MMSE scores decreased remarkably as well (t = 2.22-2.69, P〈0.05). After One month after therapy, MMSE score showed substantial increase (t = 2.26, P 〈0.05), while no significant difference was observed in terms of all items of WMS (P〉0.05). Conclusion MECT is effective and safe for depression in old age, and the induced cognitive impairment is transient.
