犬严重烧伤早期脑水肿时动脉血气的变化 被引量:2

摘要 目的 探讨严重烧伤早期动脉血气变化与脑水肿形成的关系 .方法 犬 5 2只随机分为对照组 (n=6 )和致伤组(单烧组、平衡液组和葡萄糖液组 ,n=4 6 ) .采用体表燃烧法将致伤组犬制成 5 0 % TBSA 度创面 ,于伤前及伤后 6 ,12 ,18和 2 4 h对各致伤组犬的动脉血气进行动态观察 ,并与病理组织切片对比分析 .结果 伤后 6 h致伤组各动脉血气指标开始出现变化 ,随时间迁延 ,严重烧伤早期脑水肿模型 (葡萄糖液组 )动脉血气指标变化逐渐加重 ,于伤后 2 4 h出现明显的低氧血症和酸碱平衡紊乱 ,脑组织表现为明显的脑水肿病理改变 .结论 严重烧伤早期出现的动脉血气变化对脑水肿的发生发展具有一定的作用 . AIM To explore the relationship between the changes of arterial blood gas indices and the development of cerebral edema at the early stage of severely burned dogs. METHODS Fifty two dogs were randomly divided into control, simple burn (SB), burn with balanced salt solution (BSS) and burn with glucose solution groups (BGS). The wounded dogs were inflicted with 50% TBSA Ⅲ degree flame burns. Arterial blood gas indices (pH, HCO 3 -, P aO 2 -, P aCO 2 - , Beecf) were analyzed dynamically at preburn, 6, 12, 18 and 24 h postburn hours (PBH) and compared with histopathological examination of the brain tissue respectively. RESULTS Arterial blood gas indices exhibited abnormal values in all wounded groups at 6 PBH. In the BGS group, changes of the arterial blood gas indices became evident along with time lapse. As a result, obvious hypoxemia and acid base imbalance with significant pathomorphological features of cerebral edema were observed in the BGS group at 24 PBH. CONCLUSION Changes of arterial blood gas may play a role in the development of brain edema at the early stage of severely burned dogs.
