摘要 目的了解工人长期持续接触低浓度硫化氢引起的心脏功能改变特征。方法采用定群、纵向研究方法分析研究269名低浓度硫化氢职业接触者2012年在岗期间和2009年岗前的心电图检查结果。结果 2012年心电图异常率(39.03%)高于2009年(26.77%),差异有统计学意义(P〈0.01),其中心电轴偏移、肢导低电压、ST-T改变与2009年比较,差异均有统计学意义(均P〈0.01);2012年男女心电图异常率均高于2009年,男性异常率差异有统计学意义(P〈0.01),而女性异常率差异无统计学意义(P〉0.05);2012年心电图异常率46-52岁组〉26-35岁组〉36-45岁组,46-52岁组与36-45岁组、26-35岁组分别比较,差异均有统计学意义(P〈0.01);两个年度间相比,2012年各年龄组异常率均高于2009年,46-52岁组差异有统计学意义(P〈0.01)。结论长期持续职业接触低浓度硫化氢和急性硫化氢中毒一样可以导致心脏功能损害,表现出心电图异常,可结合心电图检查结果,判断心肌损伤程度。 [Objective]To understand the characteristics of cardiac function change due to long-term exposure to low concentration of hydrogen sulfide.[Methods]The cohort,longitudinal research methods were adopted to analyze the ECG examination results of269 workers exposed to low concentration hydrogen sulfide on-the-job in 2012 and pre-service in 2009.[Results]The ECG abnormal rate in 2012( 39. 03%) was higher than that in 2009( 26. 77%),with significant difference( P〈0. 01). The electrical axis offset,limb lead low voltage,ST-t change in 2012 were significant different with those in 2009( all P〈0. 01). The ECG abnormal rates of male and female workers in 2012 were higher than those in 2009,with significant difference in male workers( P〈0. 01)and no significant difference in female workers( P〉0. 05). Rate of abnormal ECG of 46-52 years old group 〉26-35 years old group 〉36-45 years old group,compared 46-52 years old group with 36-45 years old group and 26-35 years old group,respectively,the differences were significant( P〈0. 01) in 2012. Compared between the 2 year,the abnormal rate among all age groups in 2012 were higher than in 2009,the difference in 46-52 years old group was significant( P〈0. 01). [Conclusion]Continuous long-term occupational exposure to low concentration hydrogen sulfide and acute hydrogen sulfide poisoning can lead to heart function damage,showing the abnormal ECG which can be combined with ECG examination results for determination the degree of myocardial injury.
