摘要
目的探讨三位能量多普勒超声指标与胎儿生长受限(FGR)关系。方法应用GE公司Voluson 730M三维能量超声诊断仪检测36例FGR患者及68例正常胎儿胎盘的血流指数,血管化指数,血管化-血流指数;对胎儿大脑中动脉PI值、脐动脉S/D值以及孕妇子宫动脉PI值进行分析。同时随访观察孕妇妊娠预后情况。结果 FGR组胎盘VI、胎盘FI、胎盘VFI、大脑中动脉PI分别为(14.34±4.49)、(41.04±4.53)、(6.76±2.08)、(1.56±0.39),均显著性低于正常组的(20.34±6.48)、(46.04±3.84)、(9.24±1.87)、(1.80±0.28),差异有统计学意义(P<0.05);脐动脉S/D、左子宫动脉PI、右子宫动脉PI值分别为(3.76±0.89)、(0.98±0.31)、(0.93±0.29),均显著性高于正常组的(2.33±0.29)、(0.69±0.22)、(0.65±0.20),差异有统计学意义(P<0.05)。FGR组Apgar评分为(7.34±2.39),显著性低于正常组的(9.52±1.84),差异有统计学意义(P<0.05)。FGR组新生儿窒息、胎儿窘迫发生率显著性大于正常组,差异有统计学意义(P<0.05)。结论在孕妇及胎儿血流动力学变化方面应用三维能量多普勒超声进行监测,可以判断胎儿在宫内有无缺氧状况,三维多普勒超声指标异常与FGR具有相关性。
Objective To explore the relation between the three-dimensional power Doppler ultrasound index and fetal growth restriction(FGR). Methods Voluson 730 M three-dimensional power ultrasonography provided by GE Company was used to detect the placental blood flow index, vascularization index, vascularization flow index of 36 cases with FGR and 68 cases with normal fetuses, and the PI value of fetal middle cerebral artery, umbilical artery S/D value and PI value of uterine artery were analyzed.And the pregnant women were followed up for the observation of the pregnant outcome. Results The value of placental VI, placental FI, placental VFI, middle cerebral artery PI of the FGR group was(14.34 ±4.49),(41.04±4.53),(6.76±2.08),(1.56±0.39), respectively, significantly lower than(20.34±6.48),(46.04±3.84),(9.24±1.87),(1.80±0.28) of the normal group, respectively, the differences were statistically significant(P〈0.05); the umbilical artery S/D value, left uterine artery PI value and right uterine artery PI value was(3.76±0.89),(0.98±0.31),(0.93±0.29), respectively, significantly higher than(2.33±0.29),(0.69±0.22),(0.65±0.20) of the normal group, respectively, the differences were statistically significant(P〈0.05). The Apgar score of the FGR group was(7.34±2.39), obviously lower than(9.52±1.84) of the normal group, the difference was statistically significant(P〈0.05). The incidences of asphyxia neonatorum and fetal distress was much higher than those of the normal group, the differences were statistically significant(P〈0.05). Conclusion The application of three-dimensional power Doppler ultrasound in monitoring the pregnant women and fetal hemodynamics change, can determine whether the fetus in utero has hypoxia. And the abnormal three-dimensional power Doppler ultrasound index is often correlated with FGR.
