吸氧对晚孕脐绕颈胎儿脐动脉、大脑中动脉血流阻力指标影响的临床研究 被引量:1

摘要  目的 研究吸氧对晚孕脐绕颈胎儿脐动脉 (UA)、大脑中动脉 (MCA)的血流阻力指标搏动指数 (PI)、阻力指数(RI)、收缩期血流峰值 /舒张期血流峰值(S/D)的影响,探讨脐绕颈与胎儿宫内缺氧的关系和解决途径,并寻找给氧的最佳时机。方法 运用彩色多普勒检测 118例晚孕脐绕颈 (脐绕颈组 )胎儿吸氧前后UA、MCA的血流阻力指标,和 89例正常 (正常组)晚孕胎儿UA、MCA的血流阻力指标,并进行相关分析。结果 脐绕颈组UA血流阻力指标明显高于正常组,MCA血流阻力指标明显低于正常组,后者变化更为明显。吸氧后脐绕颈组UA血流阻力指标明显低于吸氧前,MCA血流阻力指标明显高于吸氧前。结论 晚孕脐绕颈可引起胎儿宫内缺氧,主要为脑缺氧。吸氧是改善胎儿宫内缺氧的有效办法,UA血流阻力指标升高或 /和MCA血流阻力指标降低,提示胎儿宫内缺氧,应引起临床重视,并尽早给予氧疗。 Objective To study the effect of the blood resistant parameters PI,RI,S/D in the MCA and in the UA of the fetus with cord around neck when breathing in oxygen in third trimester of pregnancy.To explore that the main reason and resolving of the fetal hypoxia in the womb when cord around neck.Point out the best opportunity breathing in oxygen.Methods The resistant parameters of UA and MCA were analysis with color Doppler ultrasound in 89 normal and 118 fetuses with cord around neck in third trimester of pregnancy before and after breathing in oxygen.Make a correlation analysis.Results The resistant parameters of UA of the fetuses with cord around neck team are obviously higher than the normal team's and the resistant parameters of MCA are obviously lower than the normal team's.The MCA is more obvious.After breathing in oxygen,the UA resistant parameters of the fetuses with cord around neck team is obviously lower than before.The MCA blood resistant parameters are obviously higher than before.Conclusion The raised resistant parameters of UA and (or) the descended resistant parameters of MCA resulted in intrauterine cerebral hypoxia in fetus with cord around neck in third trimester of pregnancy.Breathing in oxygen is an effective method to ameliorate the fetal intrauterine cerebral hypoxia in third trimester of pregnancy.Fetal intrauterine hypoxia may be attached importance and should be treated with oxygen as soon as possible.
