中青年与老年脑梗死患者脑动脉狭窄分布的DSA分析 被引量:3

DSA analysis of the distribution of cerebral artery stenosis in young and elderly patients with cerebral infarction

摘要 目的应用数字减影血管造影对中青年与老年脑梗死患者脑动脉狭窄的分布特征进行分析。方法选择脑梗死患者86例,根据患者年龄分为中青年组(年龄<60岁)48例,老年组(年龄≥60岁)患者38例,应用数字减影血管造影技术(DSA)对患者脑动脉病变血管狭窄程度及分布进行分析。结果 1老年组患者在颅外动脉病变以及前循环狭窄发生率明显高于中青年组(P<0.05),而颅内动脉病变以及后循环狭窄发生率却明显低于中青年组(P<0.05或P<0.01),在脑血管病变及颅内外动脉同时受累发生率上,两组差异无统计学意义(P>0.05);2中青年组最常见为重度狭窄,发生率高于轻度及中度狭窄(P<0.05),好发部位为椎动脉开口处,发生率高于颈内动脉起始段及大脑中动脉MI段(P<0.05);老年组最常见为重度狭窄(P<0.05),好发部位为椎动脉开口处及颈内动脉起始段,发生率高于大脑中动脉MI段(P<0.05)。结论中青年患者最常受累血管为颅内血管,病变发生部位多以椎动脉起始处居多;老年患者最常受累血管为颅外血管,最常受累血管为椎动脉起始处及颈内动脉。 Objective Analysis on distribution characteristics of stenosis in young and elderly patients with cerebral infarction cerebral artery using DSA.Methods 86 cases of cerebral infarction patients were chosen,According to the age of the patients were divided into young middle aged group(aged〈60years)48cases and old age group(age≥60years)38cases,digital subtraction angiography(DSA)were used to analyze on cerebral artery lesions in patients with vascular stenosis degree and distribution analysis.Results The rate of extracranial arteries lesions and anterior circulation stenosis in elderly patients was higher than that in young group(P〈0.05),But the intracranial arterial lesionsand posterior circulation stenosis rate was obviously lower than that in the young group(P〈0.05 or P〈0.01).there were no significant difference between two groups At the same time involved in cerebral vascular lesions and intracranial and extracranial arterial incidence(P〉0.05).The most common youth was severe stenosis(P〈0.05),The primary site for vertebral artery openings(P〈0.05).The most common in old age group was severe stenosis(P〈0.05),The primary site of vertebral artery and internal carotid artery opening the initial segment(P〈0.05).Conclusion The young patients most often involved intracranial vascular,Lesion site in vertebral artery at the start of the majority.Elderly patients most often involved extracranial vascular,the most frequently affected blood vessels are vertebral artery and internal carotid artery.
